The Baruch Healthy Neighbors Partnership (BHNP) is a 3-year community health promotion program funded by the New York Community Trust and spearheaded by the New York City Department of Health and Mental Hygiene, the New York City Housing Authority and the Health and Hospital’s Corporation-Gouverneur Healthcare Services. BHNP is a pilot program to determine how effective community outreach and education efforts are in the prevention and control of diabetes, high blood pressure, cervical cancer, breast cancer and colon cancer. To do this, a team of a Community Health Worker and Resident Outreach Workers has been assembled to provide residents of Baruch Houses, a lower east side NYCHA development, with free programming and other opportunities aimed at helping program participants live healthier lives.. Ultimately, more systematic care, including systematic referral for cancer screening, should reduce health care costs by preventing or minimizing more serious disease. Our goal is to increase the use of appropriate levels of care and decrease the number of unnecessary visits to the emergency room as well as hospitalizations.
