Summary

As a member of our Clinical Team, the Intake Coordinator will oversee all new client referrals and admissions for our Residential Programs and Day Programs throughout NJ. This position requires review of referral packets, meeting potential new clients and their families, determining support needs, and placement. This position collaborates with the entire clinical team and program supervisors to ensure positive outcomes and facilitate a smooth process from referral through start date. This position is detail oriented, and requires a high level of professional communication and organizational skills.

Key Responsibilities (including but not limited to):

  • Review individual referral packets and interview potential clients as member of the clinical team to determine eligibility for Everas services.
  • Determine eligibility and make recommendations to the Everas Clinical team for admission.
  • Develop and manage the outreach program to ensure all vacancies are filled timely manner; develop and maintain excellent relationships with Support Coordinators and community resources
  • Coordinate and attend all client referral visits and preplacement activities
  • Ensure receipt of all client information required for admission; secure all adaptive equipment and medical needs required to meet client needs.
  • Responsible to visit all individuals at their current homes and day programs to ensure the available program is an appropriate fit for the referred individual and the referred individual is appropriate match for the individuals who are currently in the program.
  • Demonstrate familiarity with community contacts, including but not limited to day programs, job opportunities, recreational facilities, in order to provide information on potential community involvement to referred individuals.
  • Demonstrate professional communication with families, clients, referred individuals and representatives from referral sources.
  • Develop and complete monthly vacancy reports for Finance, HR and Executive departments.
  • Maintain accurate and timely electronic documentation for all client admission records as needed.

Experience/Skills Required:

  • Bachelor's degree in Social Work or related field
  • Proven experience in I/DD field
  • Ability to pass all background check requirements
  • Strong organizational skills and ability to work independently with minimal supervision
  • Excellent verbal and written communication skills
  • Willingness to learn
  • Valid NJ Driver's License
  • Ability to complete all required trainings including, but not limited to, CPR, ASL, etc.
