Under supervision of the Program Director, the Case Manager would assist clients in evaluating their needs, find appropriate apartment units and emergency clothing and food needs, assist clients to gain the insight and motivation to resolve problems related to domestic violence; facilitate individual, group and family counseling; client evaluation, orientation, education and treatment planning.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following but are not limited to:

Develops the service plan with client and works towards achieving goals determined. These include: Financial, housing, medical, legal advocacy; vocational educational or job placement; transferring of services for self and children to safe locations.

Provide orientation to new clients. Complete all screening and intake procedures; provide complete assessment of clients’ treatment options, and will assist with obtaining financial assistance.

Complete initial treatment plans and subsequent reviews, counseling notes, termination procedures, etc. as specified by the agency or regulatory policy.

Facilitate rapport and/or relationship via individual counseling and/or any other activities deemed appropriate. Provide a positive role model of confidence, trust, support, assistance, and reinforcement necessary to assist clients with the identification and understanding of their problems and knowledge of various methods of addressing them. 

Be available for crisis intervention, information and/or referrals for services as necessary. Observe, monitor and evaluate the progress and /or interactions of the clients and relay the information as appropriate. Observe all matters pertaining to client confidentiality. Ensure that clients understand and abide the rules and procedures and follow-up on problems and/or violations as indicated or directed.

Perform duties related to outreach and supplemental services including outreach efforts within the community. Investigate resources necessary to obtain clients goals.
