Case Managers at Daybreak, Depaul USA’s day resource center in Macon, GA, helps program participants build lives of greater dignity and independence. The Case Manager is an integral part in assessing persons experiencing homelessness and/or those experiencing housing insecurity. He/She will execute the proper intake for case management and formulate the steps and goals to move the client forward a better, more stable, place. He/She will be the primary point of contact for those seeking housing with Depaul USA Daybreak. This includes new tenant screening, new tenant orientation and education, tenant communication, visiting participants in their homes and monthly meetings with the goal to support a successful tenancy. 


Position Description:

Case Managers at Daybreak, Depaul USA’s day resource center in Macon, GA, help program participants build lives of greater dignity and independence. The Case Manager conducts interviews to determine participants’ needs and makes appropriate referrals to outside agencies to meet those needs. The Case Manager also provides direct assistance, coordinates care, and devises support plans for each participant. S/he reports to and works closely with the Assistant Director to move clients toward the aforementioned goals. The Case Manager is an integral part of Daybreak’s Housing Program. As such, the Case Manager engages in tenant screening, intake, and orientation. S/he refers tenants to educational classes that support a successful tenancy and evaluates tenants’ progress toward goals. The Case Manager is responsible for drafting case management notes for all participants and tenants that include goals set, progress towards these goals as well as recommendations that will move the participant or tenant toward higher levels of participation and stages of independence. S/he provides these services in a manner consistent with the values and strategies of Depaul USA.

 

Required Qualifications and Credentials:

The ideal Case Manager will be a professional with several years of experience in social work with vulnerable segments of the population. S/he will have experience working with individuals who faced homelessness and struggled with issues of mental illness, cognitive disabilities, and addictions, as well people who are newly housed. The Case Manager will also understand social work theories and methodologies, be familiar with relevant partner agencies, and comprehend the needs of individuals seeking services.  The Case Manager should possess the following:

  • Bachelor’s degree in Social Work or closely related field with at least two years’ experience working with populations affected by behavioral health diagnoses, addiction, and homelessness.
  • Exceptional communication, multi‐tasking, conflict management and interpersonal skills.
  • Strong computer skills with all Microsoft Office programs.
  • Ability to organize tasks, events, people, and delegate effectively.
  • Ability to exercise independent judgment when appropriate and strong decision-making skills.

·      A Georgia Driving License and a good driving record.

·      Experience working with diverse populations and the ability to meet tenants in their homes or in the communities where they reside.


Primary Duties and Responsibilities:

1              Main Tasks-

1.1          Meet with and complete intake paperwork in a timely manner for all new participants seeking case management or housing services. Track client enrollment and prepare reports, statistics, and paperwork as requested by supervisory staff and required by grants that support case management.

1.2          Offer assistance with referrals to supportive services for Social Security, SNAP, housing, mental health assistance, health insurance, and other services commensurate with participants’ needs.

1.3          Assist participants gain and maintain employment.

1.4          Establish goals and chart notes on goal progression as related to levels of participation in Daybreak’s “Stages of Independence”. Assist participants in becoming “ready” to volunteer at Daybreak and “able” to make progress from their initial intake stage.

1.5          Refer participants to existing educational groups within Daybreak and/or groups outside Daybreak as needed to meet goals. Work with Daybreak’s Volunteer Coordinator to develop and implement an enrichment program.

1.6          Refer participants to existing medical services offered by Daybreak and/or outside medical services if needed. Work with the Daybreak Clinic Manager on plans for clinic patients including applying for medical benefits.

1.7          Assist in selecting participants for tenancies in Depaul USA houses in Macon. Provide tenant screening and intake, tenant orientation, and referrals to educational classes that aid in successful tenancies. Provide additional supports to tenants including, communicating, on a regular basis, in person and/or via telephone with tenants who are also participating in active case management at Daybreak. This includes visiting tenants in person in their residences. Work with tenants to develop and monitor plans that include goals for maintaining housing. Assist tenants in building a sense of community and identifying and resolving issues occurring in their residences. Ensure all appropriate records and forms are completed by tenants.

1.8          Develop and implement a process for tenants to build and develop life skills. Develop curriculum and identify and recruit residents and employ retention techniques to ensure that tenants attend necessary life skills meetings.

1.9          Engage in outreach in the community to meet with participants, tenants, and people experiencing homelessness. 

1.10       Collaborate with other staff to develop support plans and chart notes in a timely fashion.

1.11       Collaborate with other agencies in the community and participate in coalition meetings. Build good working relationships with other stakeholders, programs, services, and agencies in the community.

1.12       Assist with opening Daybreak one day per week and assist with administrative duties (i.e., conducting tours of the facility for visitors and volunteers, supervising volunteers, greeting guests, answering the phone, and filling in at stations when needed).

1.13       Participate in staff meetings.

1.14       Participate in case management meetings to discuss client progression and areas for development.

1.15       Assist the program team with special Daybreak events or other activities that require teamwork.


2              General Responsibilities-

2.1          Adhere to all quality assurance policies and procedures. 

2.2          Stay informed of current local and national events, trends, and issues (wherever possible), that impact Depaul USA’s work with the homeless.

2.3          Strive continuously to develop the role in collaboration with the supervisor to ensure that all tasks are being undertaken in an effective and appropriate manner, meeting the strategic goals and objectives of Depaul USA.

2.4          Participate in internal/external meetings as required, attends training events, conferences, and other functions as necessary.

2.5          Participate in regular supervision and annual evaluations, identifying his or her own developmental and training needs.

2.6          Ensure adherence to all Depaul USA policies and procedures, particularly those relating to Health and Safety and the Code of Practice and Confidentiality.

2.7          Contribute to the effective implementation of Depaul USA’s Equal Opportunities Policy as it affects both the organization and its work with this client group.

2.8          Undertake the role in a professional manner maintaining a high-quality standard of work, in accordance with the Vision, Mission and Values of Depaul USA.

2.9          Undertake any other duties that may be required which are commensurate with the role.

2.10       Possess the ability to climb stairs, lift to 25 lbs.
