Position Title:             Substance Use Counselor, LMSW

Direct Supervisor:      Program Director

Job Summary: Provides counseling services including identifying, screening and assessing clients to determine the need for substance use treatment.  Provides referrals to community-based organizations and coordinates care as needed. Responsible for facilitating therapeutic group and individual sessions. Responsible for completing and maintaining detailed OASAS-compliant case records.

Duties and Responsibilities:   

·        Conduct intake interviews, biopsychosocials, and formulate treatment plans and treatment plan reviews

·        Perform individual counseling duties which include holding weekly individual sessions with clients, providing referrals to community-based resources, and developing client schedules

·        Initiate and maintain client records in accordance with Part 822 OASAS regulations and program requirements

·        Ensure accurate and timely data entry of all client information into the agency’s database; utilize electronic health record system

·        Facilitate assigned treatment groups

·        Perform drug testing as needed

·        Prepare monthly updates for court or other supervising agencies

·        Participate in treatment team case reviews.

·        Consult with other professionals in support of client’s treatment

·        Complete discharge documentation as needed

·        Participate in in-service and outside trainings

·        Participate in potential candidate interviews as needed

·        Perform other duties as assigned by supervisor

Qualifications: LMSW is required; minimum of two years’ clinical/case management experience in working with clients that have substance use disorders is required; experience working with the criminal justice population is preferred. computer literacy (word processing, e-mail, internet research, data entry) is required; ability to use an electronic health record system required; bilingual (English/Spanish) preferred.
