Description

Reporting directly to the Program Director the Child/ Family Liaison will provide counseling to clients and their family to address the clinical issues that our families face. The Liaison will meet with families in one on one sessions, make referrals to community resources as needed and work as part with the clinical team at Stone House (Brooklyn).


RESPONSIBILITIES


  • Actively engage children and families in Stone House through outreach, education, and individual sessions;
  • Facilitate referrals; provide clients with college information and financial aid and health resources; organize client participation in college tours and college fairs;
  • Develop and coordinate workshops on: parenting, child development and development of healthy relationships;
  • Establish a relationship with local health and mental health providers to develop a strong referral network;
  • Work closely with all relevant departments to assist and incorporate best practices for the engagement and support of the families;
  • Conduct weekly follow-ups with current clients and maintain up-to-date documentation of all services provided.


QUALIFICATIONS


  • LMSW required
  • Minimum of two (2) years of successful practical experience working with children and families in care settings
  • Previous experience working in a Social Services setting and working with homeless teens and young adults a plus
  • Demonstrated ability to partner with potential providers
  • Develop appropriate programming for participants
  • Excellent written and verbal communication skills
  • Must be highly organized with solid computer skills; knowledge of MS Office and Outlook preferred
  • Bilingual in English/Spanish desired
  • Ability to secure a satisfactory reference / background check and ChildLine clearance
