Director of Prevention Programs



Full Job Description



Responsible for expansion of community-based

prevention services and integration of grant-funded prevention services within

ambulatory care. Provides prevention services to patients as needed, as well as

supervision for prevention staff. Is aware of the philosophy, standards,

objectives, and policies of the Department and the Hospital Center.



* Utilizes learning resources.



* Maintains and upgrades knowledge base

consistent with requirements of the position.



* Assists in staff development and training

programs.



* Develops prevention-specific training and

education programs.



* Ensures maximal standardization of prevention

across all ambulatory settings.



* Demonstrates a professional, courteous, and

respectful attitude in dealing with patients, families, and significant others.



* Displays courtesy, tact, and patience during

interactions with all members of the hospital staff and extended community.



* May be required to perform other duties as

assigned or perform other duties when necessary.



* Demonstrates the ability to perform

clinical/technical/service/administrative tasks:



* Oversees program development as it relates to

prevention services.



* Manages all prevention-oriented grants and

personnel, including HIV and Hepatitis screening programs and sexual and

behavioral health services.



* Responsible for ensuring all prevention grant

deliverables are met.



* Builds institutional prevention capacity in

ambulatory care, both internally and through external collaborations, to meet

the goals of the hospital's community



* Improves integration of prevention services

and clinical care in close collaboration with clinical leadership.



* Monitors performance of prevention services

and ensures standardization of measures and practices.



* Provides direct prevention services to

patients, as needed.



* Collaborates with stakeholders across public

health, community service providers, and community members to develop and

implement informed prevention services.



* Develops and institutes prevention services to

develop prevention policies, procedures, and algorithms for ambulatory care.



* Provides direct clinical and administrative

supervision of prevention staff as assigned; performs performance appraisals on

same and oversees scheduling and professional development activities.



* Forms and strengthens prevention partnerships

in the community for services not provided at the medical center.



* Participates in the department's performance

improvement activities.



* Maintains patient/employee confidentiality in

the management of information.



* Ensures adherence by prevention staff to best

practice guidelines.



* Participates in management groups and

committees.



* Provides leadership in developing and ensuring

adherence to standards of care for the delivery of prevention services.



* Responsible for planning and overseeing all

prevention Continuous Quality Improvement (CQI)activities. Ensures that

prevention staff is trained on an ongoing basis in the appropriate techniques

and treatment modalities.



* With other Directors and Managers as

appropriate, develops, implements, and evaluates interventions and standards of

care to optimize the patient experience.



* Participates in the development of other staff

members.



* Meets all regulatory and annual health

assessment requirements.



* Identifies learning strengths and needs to

improve performance



* May be required to perform other duties as

assigned or perform other duties when necessary.



Position Requirements: 



* This position requires a minimum formal

education of Master's Degree and a minimum of 3 -5years of job-related

experience.



* License, LMSW/LCSW/CASAC preferred.



* Experience in program management related to

sexual health/HIV/STI/Hepatitis prevention services in underserved communities

and with disenfranchised populations. Familiarity with prevention methodology

and terminology. Three years of experience in program development, supervision

of staff, and service deliverables.
