Heights and Hills provides older adults and their caregivers with the services, support and information they need to age in place – in their own homes and neighborhoods in Brooklyn. Our mission is to promote successful aging in the community. Founded in 1971, today we are one of the largest providers of social services for older adults in Brooklyn, serving more than 4,000 older adults and their families annually. We provide case management for the homebound, information and support services to families caring for an aging loved one, as well as operating a robust volunteer program.

The Intake Worker plays a key role at Heights and Hills and is the first professional point of contact for older adults, their caregivers and other stakeholders seeking information, referrals and ongoing services. The Intake Worker will report directly to the Director of Programs and work closely with other Program Directors and other program team members to ensure high quality service provision.

Role

The Intake Worker is responsible for fielding calls requesting services, seeking information or guidance. The Intake Worker will assess the needs of each caller after a thorough phone intake and makes referrals and linkages to community resources and/or to Heights and Hills programs.  When Heights and Hills has a waiting list for assignment to case managers, the Intake Worker is responsible for maintaining and monitoring the wait list.

  Responsibilities

  • Respond to incoming inquiries from individuals, caregivers, social service agencies and healthcare providers
  • Conduct a thorough assessment of client needs via a phone intake
  • Make referrals to appropriate H&H programs
  • Make additional referrals and linkages to community services when appropriate
  • Provide information and assistance to callers
  • Maintain accurate statistical data
  • Cultivate relationships with community providers to serve as a network for referrals and linkages
  • Oversee a waitlist for case management services; may involve ongoing client contacts
  • Work together with Director of Programs and CM Director regarding case assignments
  • Oversee a small caseload of APS referrals
  • Perform other duties as required

Required

  • LMSW with a minimum two years of experience;
  • Experience with the elderly population
  • Knowledge of social work theory, interviewing principles, community resources and public benefits eligibility.
  • Strong clinical and assessment skills
  • Excellent organizational skills
  • Ability to respond to crisis situations
  • Excellent computer literacy – Windows, Microsoft Office 

Preferred

  • Bi-lingual/cultural competency in Spanish, Chinese or Haitian Creole. 
  • Prior experience with the STARS program
