The Actors Fund, a nationwide human services organization that helps all professionals in performing arts and entertainment, seeks a dynamic Social Worker to provide social services to residents 60+. The Rodney Kirk Center is a residence based social services program offering services and activities to help older residents stay socially engaged, maintain their independence allowing them to safely age-in-place.
RESPONSIBILITIES
This position provides direct services to residents 60+, their families and significant others including intake and assessment, intensive clinical case management, individual and group counseling as well as coordination of discharge planning with hospitals and long-term care facilities to ensure safe discharge planning for residents returning home. The social worker will also manage annual outreach to residents who are 75+ years. Collaborating with the Activities and Volunteer Coordinator, this position will have opportunities to develop and present seminars that address social and health related needs particular to older adults as well as facilitate events.

This is a full-time salaried position based in The Rodney Kirk Center, Manhattan Plaza.
