This position is fast-paced and diverse. We are seeking compassionate individuals with a strong work ethic and who believe in providing high quality services to those that they serve. Services are wide-ranging and include but are not limited to: developing care plans, mediation, client advocacy, developing client budgets, resource coordination, etc. This position requires some travel in the Southern CA area but is mostly campus-based. Individuals who apply must value direct client interface, have a strong social service background working with the geriatric population, is detail-oriented and is computer and mathematically proficient.

The Care Manager will provide Care Management duties through the Shared Housing and Masonic Senior and Family Outreach Service Programs, under the Department of Masonic Outreach Services. The Care Manager will be responsible for supporting the programs through the coordination and management of care services for members of the fraternity. The Care Manager will assist clients with problems associated with aging and finances, benefits location, eligibility, application, and coordination. They will also assist clients with issues of economic hardship, job placement and job search efforts, housing, social service programs, counseling services, and financial guidance.


ESSENTIAL FUNCTIONS (SHARED HOUSING PROGRAM)

· Provide ongoing Care Management to clients who reside on the campus-based Shared Housing Program. These duties will be provided on site. Additional care management will be provided to clients on the Masonic Outreach Services programs as needed by telephone and on-site at client’s homes, RCFEs, or wherever the clients are located. Length of travel is usually day trips, or one occasional overnight. Rental car, hotel, and airline costs provided by Masonic Outreach Services for travel outside of local area. Local travel will be done in Care Manager’s own vehicle with mileage reimbursement.

· The Care Manager will help develop and implement individualized service plans to address all physical, psychosocial, environmental and safety factors to ensure that all clients are appropriate for occupancy

· Works in conjunction with the Program Manager to ensure that the Facility operates in accordance with all applicable regulations and Facility-specific policies and procedures

· Helps coordinate client admissions, ensuring all required paperwork is completed in a timely and accurate manner.

· Develops, in conjunction with the Program manager, client service plans that address the specific needs and preferences of each client; ensures that revisions are made on a regular basis and as significant changes occur.

· Includes interacting with the client, their family and appropriate support services.

· Coordinates the transfer of clients to more appropriate care settings, as needed either on a temporary or permanent basis. Helps to oversee any necessary eviction proceedings.

· Ensures the facility building and grounds are maintained in a clean, attractive and safe condition and all needed preventative maintenance is performed in a timely manner.

· Includes planning and implementation of social and recreational program that addresses the social, spiritual, emotional, and physical needs of the clients.

· Ensures that the client’s physicians and others support providers as deemed appropriate are kept informed of changes in client’s conditions and service needs.

Develops client budget based on current information and according to program guidelines. Responsible for monitoring and adhering to this budget, and making timely database updates to navigate budget adjustments and funding requests.

· Reviews and prepares reports, check requests, stipend adjustments, and correspondence in accordance with program guidelines, and the respective due dates.

· Updates and maintains accurate information on the online access database, demonstrating a thorough knowledge of the system.

· Investigates and responds to any concerns or complaints expressed by clients, families, and/or staff.

· Works with clients to problem solve and identify solutions to problematic behaviors

· Report all unusual incidents, including client-to-client incidents


ESSENTIAL FUNCTIONS (MASONIC SENIOR & FAMILY OUTREACH)

· Performs assessment to evaluate physical, functional, financial, emotional and environmental status of clients on the Masonic Outreach Programs.

· Develops individual care plans for these clients, providing appropriate and adequate resources to support their on-going needs as it relates to their economic, psychological and physical well being.

· Maintains a regular visit schedule with clients

· Informs clients and appropriate family members of all public benefits to which they may be entitled and provides guidance and / or facilitation in obtaining these benefits in a timely manner.

· Maintains ongoing open communication among all care providers, lodges and family, utilizing diplomacy, respect, and professionalism at all times.

· Uses independent judgment, based on experience, to monitor and resolve clients’ issues to ensure they are receiving a level of care appropriate to their evolving status. When necessary acts as an advocate for client to ensure fair and equitable outcomes.

· Coordinates placement and relocation services for clients when moving to assisted living, or other housing option.

· Builds and maintains relationships with community resources and support services for the clients. Keeps informed of care trends, new programs and resources and acts as a source of information for clients and families.

· Performs related duties as assigned.

SKILLS, ABILITIES AND EXPERIENCE

· Knowledge of social services and benefits programs available to families and children, as well as seniors, such as Medicare, MediCal, SSI, SSDI, TANIF, etc., and experience with assisting others to obtain these benefits.

· Knowledge of healthcare insurance options.

· Familiarity with Social Security, pension plans, and other income sources for retirees.

· Experience with accessing Mental Health services.

· Experience with accessing services through the public school system.

· Must possess strong time management and organization skills: prioritizes and plans work activities and uses time efficiently.

· Must possess strong communication skills to communicate effectively with seniors, families, vendors, and other related professionals.

· Models healthy interactions and respectful communication with clients

· Relates to clients objectively and fairly,

· Must be able to exercise independent judgment, demonstrate good assessment skills and relate effectively to the elderly, children, families, and professional associates related to these groups of individuals.

· Effective problem-solver and critical thinker

· Intermediate computer skills in Microsoft Outlook, Word, Excel and SalesForce.

· Ability to perform basic to moderate arithmetic calculations.


EDUCATIONAL REQUIREMENTS/LICENSES/CERTIFICATES

Any combination of education and experience that would provide the necessary knowledge and abilities listed, typically:

· Bachelor’s degree in Social Work, Psychology, Sociology, Gerontology, or related field from an accredited college/university, and / or relevant experience in care management for families and older adults. Masters degree strongly preferred.

· Must possess and maintain a valid California driver’s license without restrictions.
