CASE MANAGER OPPORTUNITY

Full-time with full benefits: 35-40 hours per week

Hourly Pay $16-$35 an hour depending on qualifications and experience

Job Overview:

●      Advocate and partner with other staff and community stakeholders to build trauma-resilient communities by effectively assisting with the recruitment, advisement/counseling, and career placement of Louisiana Green Corp program participants. This position will assist with documenting participant progress through all cycles of the program from interested program applicant to program graduate.

●      The Case Manager will serve as a subject matter expert on Mental Health, Substance Abuse, and/or Trauma-Informed Care of each participant.

●      The Case Manager will be a resource for professional development to all staff and families around social-emotional development, resiliency, positive behavior support, self-care, maternal depression, brain development, substance abuse, and trauma-informed approaches.

●      Actively contributes to the sustainability planning of the organization despite changes in leadership, policy, funding, and staffing.

 

Job Duties

-Assists with referral, intake, and enrollment procedures for individuals entering the Louisiana Green Corps Construction and Conservation Corps program by updating files and providing information on the status of program participants in required database systems.

-Completes Needs Assessments and Individual Development Plans with each participant on a biweekly basis. To ensure that participants adhere to their Individual Development Plans and the program can meet the participant’s needs.

-Provides confidential counseling and referrals for services based on individual needs. Keeps appropriate notes and notifies supervisors of participants who are at risk or non-compliant as soon as possible.

-Develops strategies and plans to correct behaviors and assist participants in developing coping skills and behaviors to navigate challenges encountered while matriculating as participants.

-Conducts follow-up calls and communicates with graduates, connecting them to opportunities and resources to best meet their needs. Acts as a liaison and maintains lists of resources utilized to assist with participants’ positive growth and development.

-Maintains a database of resources and contacts to assist with participants’ needs. Develops objective data reports to target intervention strategies as necessary. Assists with data tracking, grant monitoring, and reporting as assigned.

-Teaches classes on employability skills, soft skills, and others as needed. Develops a calendar of activities to assist with counseling participants individually and as a group. Creates opportunities to address the unique needs of each program participant to enhance success and outcomes.

-Completes other duties as assigned in a timely, organized manner.  Provides written statements as needed to ensure that participants are provided with assistance promptly.

 

Technological Skills

Basic Microsoft operating systems, navigation, outlook, word, excel, and PowerPoint. Ability to assist with the production of content for newsletters, annual reports, board reports, grants, or social media accounts as needed. 

 

Competencies, Skills, and Abilities

-Experience building trust and working with communities. The candidate must have the ability to exercise good judgment and interact with all stakeholders with integrity and professionalism.

-Demonstrated understanding of Louisiana Green Corp target participants.

-Demonstrated ability to perform job responsibilities independently with minimal supervision.

-Demonstrated ability to deal with participants who may be uncooperative and overwhelmed with emotional stress.

-Demonstrated ability to deescalate conflict and mediate misunderstandings among participants and in the workplace if necessary.

-Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA, and other federal, state, and local standards, with or without accommodations.

-Ability to maintain regular, punctual attendance consistent with ADA, FMLA, and other federal, state, and local standards.

 

REQUIRED QUALIFICATIONS:

· Bachelor’s Degree in Psychology, Human Services, Education, or related field. Preference for candidates with or working towards a Masters in Social Work or Masters in Counseling.

-Candidates may subsite degree requirements with an Associate of Arts degree in the Social or Behavioral Sciences, Criminal Justice, or Human Services from an accredited college or university and two years of experience providing direct services to similar populations.

-Candidates may substitute US Armed Forces Military experience as a commissioned officer in Social Service classification or Social Services specialty Codes in the Social Sciences, Psychology, or Welfare field for four years or more for educational requirements for this position. 

-Valid Louisiana Driver’s License, clean driving record, and reliable transportation to attend meetings and deliver documents to agency partners.

-Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, and employment/education verification. Applicants will be considered based on their qualifications for the position. Louisiana Green Corp
