Position Summary:   

Responsible for managing all new intakes starting with the referral and ending with admission decision and case assignment. Must be able to engage patients across the life span. Engage community partners in making referrals. 

Primary Responsibilities:   

• Provide and coordinate all assessments in the intake process, respond according to individuals seeking treatment that are high risk.  
• Meet productivity benchmarks.  
• Serve as liaison and advocate with other agencies.  
• Outreach in the community and develop referral partnerships.  
• Assess the patient’s appropriateness for admission and manage the scheduling of all pre-admission appointments; make referrals for individuals needing different level of care and maintain waitlist.  
• Complete all required documentation concurrently in a timely manner in the agencies EHR, including assessments, notes, treatment and safety plans.  
• Use time efficiently and personally well organized.  
• Perform other tasks and special projects as assigned.   

Qualifications:  

• Licensed Clinical Social Worker (LCSW) with one to two years’ experience. Bi-lingual is a plus.  
• Strong commitment in ongoing learning and training in the areas of abuse, domestic violence, child welfare, SUD and modalities of treatment.  
• Effective written, verbal, organizational, interpersonal skills and computer literacy required plus knowledge of evidenced-based practices.  
• Must have clinical skills, including ability to assess individuals for proper treatment and evidence of trauma and crisis.  
• Must be able to establish priorities and a course of action for handling multiple tasks.
