Job Path is seeking a Registered Nurse for the full time Nurse Case Manager position. The Nurse Case Manager will provide medical services in response to health problems, promote health and safety; provide case management services; collaborate with clinical and managerial staff, and conduct staff trainings. The full time Nurse Case Manager position requires, but not limited to, the review of various assessments, logs, and follow-up where necessary; ensuring all medical appointments; tests and lab work are completed as required; providing 24 hour on-call phone coverage for emergency situations; and assisting with the AMAP certification process. The full time Registered Nurse will serve as a valued member of an interdisciplinary team.


Responsibilities include (but are not limited to) the following tasks:

  • Assessment, planning, implementation and supervision of medical care for the individuals we serve.
  • Provide and supervise comprehensive nursing services and interventions to assure that the health care needs of individuals receiving services are met.
  • Assure that the medical needs of the individuals are addressed by working with physician(s) as appropriate. Follow through with the provision of nursing care by monitoring nursing and related services.
  • Develop a Nursing Care Plan for each individual and ensure the plan is reviewed and revised as needed.
  • Maintaining 24 hour on-call capabilities for report on individual’s status and interventions as needed.
  • Provide for the individual’s continued health, welfare and safety by direction, coordination, and supervising the individual’s medical care plan along with the implementation of all agency and OPWDD policies and procedures that are related to the Rights, Care, Treatment, and Protection of all individuals.
  • Identify, assess and report circumstances occurring at the time a person experiences a seizure, injury, illness, behavioral episode, and/or any other medical or emotional difficulty an individual may have and implements therapeutic interventions accordingly. Treatment team responsibilities including annual, semi-annual and weekly medical meetings; both off and on site.
  • Collaborate with the managerial team that all staff are medically trained to meet the individual's needs by monitoring, training and evaluating all med-certified staff as per Job Path and OPWDD guidelines.
  • Provide oversight and training of direct care staff in monitoring medical systems designed to ensure the highest quality of care.
  • Responsible for participating in pre-admission and treatment team meetings; maintaining case files; dispensing medication(s); monitoring vitals and performing other nursing services as deemed necessary.
  • Conduct nursing training for program staff, as well as general health/hygiene, infectious disease control/prevention, CPR/First Aid training and other related topics for agency staff development.
  • Update MARS monthly and as needed.
  • Certify staff in AMAP
  • Schedule all medical follow ups and ensure all visits are done in a timely manner
  • Ensure that all appointments are recorded on the calendar and consult sheets, and that the clinic summary visit form is completed properly including doctor’s recommendations.
  • Ensure all prescriptions are up to date, labels of medications match the prescriptions and formularies are in the MAR binder.
  • Ensure all first aid and personal hygiene supplies are available at all times, and that the inventory is maintained at a suitable level.
  • Maintain accurate report of weight, fluid intake, menses chart, and other significant symptoms/health-related occurrences. As well as behavior charts as necessary.
  • Teach and reinforce principles of human growth and development, including human sexuality, as determined by each person’s developmental level and program plan.
  • Actively participate at staff meetings by communicating observations concerning progress and reaction to various therapy programs.


Qualifications:

Education: BA in Nursing, Licensure as a Registered Nurse in NYS, meeting all education, testing and experiential requirements identified by the NYS Office of the Professions

Experience: Registered nurse, 1-2 years OPWDD or mental health experience preferred.
