Description

The Case Planner is a professional staff position in the program providing comprehensive case management services for all assigned youth and their families. The position is highly interactive with the youth and their families, the program staff and community of origin. The Case Planner conducts face to face contacts with the youth and family in accordance with OCFS/Graham requirements; Completes assessments and reports as required by program, courts etc.; Arranges and monitors family visitation; acts as liaison with family members and the youth; co-creates comprehensive service plans with the youth and family; serves as a liaison between the agency and courts, school and community based agencies on behalf of the youth; Maintains regular contact with significant collateral contact; timely submission of up to date progress notes and reports in accordance with OCFS/Graham standards Submits all necessary reports in a timely fashion. Other duties as assigned.

 

 

Qualifications:

Bachelor’s degree in social work or related field from an accredited college or university preferred. A record of success and achievement in youth related residential child care work; Knowledge of child and adolescent development, professional child care practice and relevant clinical issues; knowledge of OCFS/youth detention requirements; Must possess a valid state driver’s license

 

An applicant for employment will not be hired if his or her driving record reflects any of the following infractions; some examples include but are not limited to:

•         Suspended, revoked or otherwise invalid license

•         Reckless , high risk or otherwise unsafe driving behavior within the past 3 years

•         6 or more points on license 

 

 

IT IS A PLUS IF YOU:

Are Bilingual; Have knowledge of OCFS regulations and mandates; Previous experience in: community organizing, political campaigns and/or customer service ; Have prior experience with the Juvenile Justice or Prison systems.
