We are currently recruiting for a Housing Retention Case Manager to support successful and enduring transitions to permanent housing.  The Housing Retention Case Manager will work with women who are moving/have recently moved from WPA residences (Sarah Powell Huntington House family shelter and Hopper Home adult shelter) into permanent housing.  The case manager will partner with women who have secured permanent housing with the assistance of WPA staff from any program.  The case manager will spend a majority of work hours meeting with clients in their homes throughout the five boroughs of New York City.

Responsibilities will include (but are not limited to):  

·       Provides guidance and instruction to family members on independent living skills and housekeeping individually, in family sessions, or in groups 

·       Refers clients to WPA and other provider programs for medical care, mental health, and family counseling services, drug treatment/relapse prevention, employment, and vocational or remedial education, and any additional needed assistance

·       Visits clients to assist with home set-up and to accompany client in exploring and becoming familiar with neighborhood, including essential merchants and services.

·       Assists clients with developing and tracking budget and spending, managing bill payments, etc.  

·       Assists clients in identifying and establishing service relationships with required services, such as medical and mental health care, sobriety support, child-care.

·       Participates in client case conferences 

·       Escorts clients to court and advocate on their behalf with external providers and at fair hearing meetings 

·       Conducts reassessments of client needs and strengths toward housing stability and independent living using information gathered through interviews, observations of family and household, and conferences with other providers  

·       Maintains documentation of visits and contact 

·       Provide aftercare services to participants and families for 6-12 months to ensure housing stability and permanency.

·       Performs any other department or agency related duties or special projects as directed by supervisor

·       Participation in all external and internal staff trainings. 


Qualifications:

·       BA/BS/BSW preferred; comparable work experience will be considered 

·       Bilingual (Spanish-English) a plus 

·       At least 2 years of documented relevant social services experience, preferably in housing/homeless programs

·       Familiarity with available affordable housing options in NYC (City FHEPS, FHEPS A and B, etc.)

·       Experience/familiarity with DHS data systems

·       Knowledge of public entitlement programs and systems required

·       Strong computer skills required

·       Excellent written and verbal skills
