Provide outreach, advocacy, case planning, service coordination and case management for families with significant barriers to employment who are referred to the TANF Case management program. This position requires intensive client-centered, in-home and/or community based case management including client assessments, Family Development Plan, assistance in life skills as needed; referrals to appropriate community services based on barriers to employment and family needs.

Establish and maintains relationships with local/regional staff from state departments, workforce development boards and DSS/DOL case workers. Must demonstrate ability to work with culturally and linguistically diverse families; ability to work independently with a flexible schedule to include non-traditional hours (early evenings/and possible weekends). Must demonstrate computer literacy, ability to work in a Windows environment and utilization of Efforts to Outcomes (ETO) web-based management system for case work documentation; and utilization of mobile devices (cell phone and tablet).

Candidates must possess a valid driver's license, insurance and dependable car.

BA/BS in a human service field with two (2) years relevant experience (preferred), Associates or equivalent with over five (5) years of directly relevant experience may be considered in lieu of BA/BS.
