The Intake Specialist works closely with the medical providers from Harlem United’s clinics to enroll patients with upcoming visits to primary care. He/She will be responsible for completing the enrollment package for patients in need of Health Home services and with the assistance of the providers and the no show reports will identify prospective patients eligible for Health Home services.
Essential Job Functions
The following duties are mandatory requirements of the job:
q Participate in daily huddles for assigned providers in Primary Care.
q Keep track of all referrals made by the clinics as well as outcomes.
q Complete the intake package for patients in need of Health Home services.
q Work in collaboration with the medical team and Business Development to enroll 5 new clients per week.
q Verify eligibility criteria for prospective clients before completing the Health Home enrollment package.
q Conduct outreach to potential clients identified by the clinic.
q Attend bi-weekly supervisory meetings.
q Complete data entry as needed.
q Provide excellent customer service at all times.
The following duties are to be performed as assigned by the supervisor:
q Participate in CQI activities.
q Participate in multidisciplinary task forces, committees, and projects.
q Perform other related duties as required.
¨ High School graduate or equivalent plus 1year relevant experience or an Associate Degree.
Special skills and knowledge
¨ Excellent computer skills necessary.
¨ Demonstrated ability to use word-processing, or data base programs.
¨ Excellent customer service skills.
¨ Good oral and written communication skills.
¨ Good organizational skills.
¨ Good interpersonal skills.
¨ Bilingual Spanish required
