This position reports directly to the Case Management Supervisor and works along with the Case Management team.


Housing Initiatives (HII) is dedicated to housing tenants who have experienced homelessness and are living with severe and persistent mental illness. HII is seeking to hire an experienced community mental health case manager to work with our tenants who are newly into housing after experiencing chronic homelessness and are still living with a co-occurring severe and persistent mental illness.


This position will be part of a Case Management team that uses Housing First, harm reduction, and ACT principles to assist our tenants in maintaining their housing and reducing recidivism back into homelessness while also decreasing their involvement with psychiatric ER visits and/or contacts with the criminal justice system.


Housing Initiatives understands that many of our tenants experience co-occurring substance and/or alcohol use issues that can be problematic. HII is a reality-based agency and aware that abstinence may not always be the goal for every tenant and therefore we are seeking a case manager who has experience in helping people who are not interested in sobriety at this time. This candidate should also have experience in helping people develop motivation to consider change and/or reduction in their substance use and be comfortable having direct conversations about safety and high risk behaviors. This candidate shall also be aware of the resources available in our community to help people develop supports for making changes.


Qualifications/Experience

·        Experience providing services to clients who have severe and persistent mental health challenges

·        Familiarity applying trauma-informed care and harm reduction models to case management roles

·        Experience with AODA and mental health systems in Dane County

·        CCS certification preferred or ability to become certified

·        CSAC or SAC-IT preferred

·        Must have a valid driver’s license and their own vehicle

A solid working knowledge of Housing First principles


We are seeking someone with an academic background in social work, rehabilitation psychology, human ecology and have a preference for Master's-level clinicians.

The candidate needs to be comfortable with transporting people in their vehicle. The expected work hours are generally Mon-Fri 9am-5pm but can be flexible based on our tenant's needs. This position is full time.
