For 30 years, Bridges Outreach, Inc. has existed as a non-profit organization providing food, clothing and other necessities to the homeless and indigent people of New York City, Newark, Irvington and Summit.
Through its "Project Connect," program located in Newark, NJ, Bridges continues to provide intensive case management by linking individuals and families with services, connecting to shelters and other temporary and permanent housing options, and assisting individuals with obtaining identification in order to access services. In four short years, the Project Connect program has serviced over 5,000 individuals with essential services.

Bridges Outreach seeks an innovative team member and compassionate intensive case manager to provide intensive case management to identified homeless and low-income individuals and families by processing all referrals for shelter/housing, employment, legal, medical and supportive services. The goal is to provide a continuum of care that will lead to better health and ultimately housing. Case Managers report to the Program Director.
RESPONSIBILITIES of the Intensive Case Manager, Project Connect:
Provide case management services including:
Orient clients as to the services and requirements for services available
Conduct initial intakes and assessment to determine clients emergency
and immediate needs
Develop service/goals plans, monitor and evaluate clients progress
Conduct supportive counseling
Participate in field visits and transport clients to medical/housing/mental health/substance abuse treatment appointments
Record all counseling interviews
Conduct or refer clients to healthy living and stability workshops (i.e.
budget, fiscal management, daily life skills, tenant rights and other
related topics)
Develop and maintain links to social service agencies
Maintain contact with referral sources
Act as a liaison between clients and agencies rendering services to
clients within your caseload
Maintain legible and complete files
Participate in weekly case conferences; attend staff meetings and
trainings as required
Work in tandem with volunteers and interns
Other duties as assigned
REQUIREMENTS AND SKILLS
MSW and 1-2 years' experience with direct consumer services
Experience working with mental health population
Supervisory experience (plus)
Experience working in an environment with high consumer volume
Experience counseling a diverse population a must
Have the ability to build a rapport with clients
Understand economic and sociological barriers
Good writing and verbal skills
Bi-lingual English, Spanish/Portuguese a plus
**80% client-centered travel required
