Depaul House operates on a 24/7 basis and provides accommodation and support to homeless men who are seeking employment and income to exit homelessness. Residents can live in the program for 12 months. The Program Counselor is responsible for conducting assessments to determine client needs and plans for obtaining needed services. The Program Counselor will create housing discharge and savings plans for clients and assess progress towards goals. The Program Counselor will demonstrate direct assistance, coordination of care, and linkage to the appropriate services while developing support plans for residents, and he/she will report to and work closely with the Associate Director, moving clients toward greater stability. The Program Counselor is responsible for documenting case management notes, activities, progress, and recommendations as well as assisting clients to access referred services and move toward independence and more stable housing.

Primary Duties and Responsibilities:

Main Tasks

Provides guidance and support for project residents.

Ensures that the practical needs of residents are met (in conjunction with volunteers); i.e. providing food, laundry, clean bedding, etc.

Ensures a safe and secure environment and maintains high operational standards within the project.

Assesses the needs of residents and refers them on to appropriate agencies and social service programs.

Develops support plans to coordinate services for a number of residents on a regular basis (at least once a week) recording the work appropriately, advocating for them, organizing their files, and updating the supervisor and the project team on their progress as required.

Builds professional relationships with all residents and assists them with difficult or problematic situations in a sensitive manner.

Assists with reception and administration duties as scheduled.

Ensures adherence to all quality assurance policies and procedures.

Team Work

Works with other members of the staff while on shift and makes decisions consistent with those of other Program Counselors, as agreed by the project team.

Transitions pertinent information to other staff at the end of a shift.

Shares responsibility in supporting volunteers.

Other

Assists the project team in the preparation of reports, statistics and other papers which may be required by the Philadelphia Director.

Conducts data entry and maintenance on a computerized monitoring system.

Conducts tours of the facility for visitors and volunteers.

Builds up good working relationships with other projects and agencies.

Stays informed of current local and national events, trends, and issues (wherever possible), that impact Depaul USA’s work with the homeless.

Ensures all appropriate records and forms are completed by residents.

Maintains all records of work necessary for the smooth running of the project.

General Responsibilities

Strives continuously to develop the role in collaboration with the supervisor to ensure that all tasks are being undertaken in an effective and appropriate manner, meeting the strategic goals and objectives of Depaul USA.

Participates in internal/external meetings as required, and attends training events, conferences and other functions as necessary.

Participates in regular supervision and annual appraisal, and helps in identifying your own job-related development and training needs.

Ensures that all Depaul USA policies and procedures are being adhered to, particularly those relating to Health and Safety, Code of Practice and Confidentiality.

Contributes to the effective implementation of Depaul USA’s Equal Opportunities Policy as it affects both the organization and its work with this client group.

Undertakes the role in a professional manner maintaining a high quality standard of work at all times, and to work always in accordance with the Vision, Mission and Values of Depaul USA.

Undertakes any other duties that may be required which are commensurate with the role.

We operate an 8-bed medical respite center in the same building. Philadelphia is also home to Immaculate Cleaning Services (ICS)—a subsidiary of Depaul USA and for-profit janitorial company whose profits flow back to Depaul USA. In October 2015, Depaul opened St. Raymond’s House, a permanent supportive housing program for 27 men and women with chronic health conditions.
