TITLE: Case Manager

DEPARTMENT: Family Services

REPORTS TO: Director of Family Services 

HOURS: 20 hours per week

This position is the main point of contact between Heart of Los Angeles and our families who have been referred by their program for additional support. The Case Manager maintains an up to date catalog of external and internal family resources, promotes them generally, and uses them in consultation with families as necessary. The Case Manager assists families in accessing resources when navigation of processes poses a barrier - identifying these families in consultation with the Director of Family Services. The position is also responsible for assisting in coordination of Parent Workshops and Events. 

Job Responsibilities

  • Maintains Family Resource Guide - in English & Spanish
  • Ongoing research, vetting & relationship building with outside service providers & governmental agencies.
  • Provides support and advocacy for families referred for crisis or support services.
  • Responds to referrals from program staff and conducts initial interview with family members to assess strengths and challenges.
  • Helps identify relevant resources and provides information for accessing.
  • Works with referred families to develop a stability plan and provides follow-up support as needed.
  • Advocates for members and families when appropriate.
  • Coordinates distribution of in-house resources.
  • Coordinates referrals from program departments for in-house resources.
  • Maintains records of families receiving in-house support.
  • Coordinates distribution efforts.
  • Assists in “Sponsor-a-Family” efforts, ensuring equitable distribution and sensitivity in donor relations around family dignity and privacy.
  • Maintains accurate records of all families referred for support services.
  • Keeps department heads apprised of coordinated responses for families with students in multiple programs.
  • Convenes relevant staff members for collaborative decision making when appropriate. 
  • Generates reports regularly.
  • Promotes resources of general interest within our parent community.
  • Assists Director of Family Services in Coordination of Parent Workshops & Events
  • Provides Spanish translation as necessary for parent communication.

Skills and Qualifications: 

  • Bachelor's Degree from an accredited college or university in social services, psychology or related field,  BSW strongly preferred.
  • Minimum two years experience working in non-profit social services. 
  • Experience working with urban youth and families, must demonstrate sensitivity to multicultural issues, and be comfortable working in a multi-cultural urban environment.
  • Working knowledge of Trauma Informed Practices.
  • Strong organizational skills, specifically database management & electronic communications strategies.
  • Excellent communication skills - balancing empathy & relationship building with clear information delivery.
  • Able to work independently but also function as part of a team.
  • Bilingual - Spanish/English, strong writing skills in both languages
  • Impeccable ethical principles and practice, especially around confidentiality and reporting.

The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus.

The noise level in the work environment is usually moderate. The building environment does not provide air conditioning and heat in all areas. Outdoor work is expected in some cases.
