Seeking Full-Time Case Manager for our Department of Homeless Services Program.

 SUMMARY OF CASE MANAGER

The Case Manager is supervised by the Social Work Supervisor and is responsible for ongoing needs assessment, individual treatment plans, implementation, support, information, referral and advocacy for individuals who are in the Providence House DHS program. Keeping clear and accurate files and records is essential to this position. The position will require occasional evening and weekend work hours.

 REPORTS TO:   SOCIAL WORK SUPERVISOR

 RESPONSIBILITIES:

 Client/Service Delivery

  • Responsible for assessing the needs of the resident, preparing treatment plans ensuring needed services is provided to the resident.
  • Meet weekly with residents to discuss progress on treatment goals, development of life skills and provide supportive environment for the discussion of life issues, which includes housing entitlements, employment, counseling, substance abuse programs, and education and parenting skills.
  • Conduct intake interviews, provide general information, gives tour of house, explain house rules, expectation of behavior, and emergency procedures.
  • Arrange meetings with housing specialist and employment specialist that will assist residents in securing housing, employment and other services.
  • Prepare resident to negotiate with agencies, i.e. housing, employment, welfare, education.
  • Complete bi-weekly independent living plan.
  • Maintain ongoing contact with DHS concerning each resident’s progress, and participates in regular DHS meetings as scheduled.
  • Enter and maintain current client information in the DHS CARES case management system.
  • Maintain a current list of community resources and ensure that residents receive needed services.
  • Advocate and escort clients to outside service agencies when necessary.
  • Maintain accurate case records and prepare required forms and reports for outside agencies
  • Participate in individual supervisory sessions and case reviews with the Shelter Director.
  • Attend all mandatory training, workshops and meetings as required.
  • Meet with DHS Program staff, Core Community and other pertinent staff on a regular basis to discuss matters of mutual concern.
  • Performs other duties as assigned by Social Work Supervisor.

Team/Staff Relations

  • Work as a Providence House team member with all staff in order to implement the mission and core values of the agency.
  • Attend regular internal and external agency meetings.
  • Work as part of a team to plan and develop resident programs.

Administrative

  • Keep accurate, up-to-date documentation on all clients. Including but not limited to, treatment plans, case notes, etc.
  • Complete all paperwork in a timely and accurate manner that ensures payment to the agency from various sources.
  • Assists with any and all data collection systems.

Travel

  • Requires travel with clients to outside agencies when necessary.
  • Position requires travel to Providence House residences and outside meetings, workshops and conference with outside agencies.

REQUIREMENTS

  • Bachelors’ Degree, Social Work or related field.
  • Two (2) years of case work experience
  • Experience working with the homeless population.
  • Excellent verbal and written communication skills and the ability to work independently.
  • Excellent interpersonal and organizational skills
  • Competent technology & management skills, including proficiency with using Microsoft Office; and client database management.
  • Required travel to our Brooklyn residences as well as throughout the NYC area.

WORK SCHEDULE

The Case Manager position is a full time, salaried position. Hours worked will generally be weekdays 9:00a.m. - 5:00p.m., at least one week night a week is required. Availability on weekends and week night as needed. The office locations for this position is Brooklyn, NY.
