Position Title: Neighbors First Permanent Housing Support Case Manager

Provide housing and supportive services to individuals experiencing homelessness or formerly experiencing homelessness, identified as the most vulnerable in DC, who are enrolled in the government funded Department of Human Services Permanent Supportive Housing (PSH) Program. The Neighbors First program follows a Housing First Model and philosophy with a scattered site approach. The Case Manager works to empower consumers to rebuild their lives, relying on their own strengths, and using wraparound resources in a holistic, consumer-driven process.
- Bachelor’s degree in a human service field and 2+ years of case management experience
- Understanding of adult rehabilitation values and principles
- Experience with individuals experiencing homelessness, addictions, mental health, and co-occurring disorders
- Strong written and verbal skills
- Commitment and passion to end homelessness
- Ability to work in a culturally diverse environment, work well under pressure, and display flexibility
- Ability to work as a team player
- Valid driver’s license with insurance and motor vehicle
Provide supportive services to homeless adults with mental illness, addictions, and complex medical needs.
Provide services in the community and consumers’ home, spending approximately 80% of time in the field.
Foster wellness and independence, demonstrating empathy and flexibility in responding to consumers’ needs and requests.
Engage consumers and assess individual needs, preferences and strengths.
Develop, implement, and revise individual service plans with each consumer on caseload
Monitor and assess consumers’ medical and psychiatric needs, medication management, and assist in locating/selecting wraparound services.
Assist consumers in securing adequate housing resources, navigating the housing process, and act as a liaison with landlords and other community stakeholders.
Collaborate with external agencies providing services to consumers.
Maintain monthly and quarterly reports and consumer’s electronic records in the Housing the Homeless Database.
Attend professional trainings and conferences as needed.
Attend and participate in regular staff meetings and supervision.
Perform all other duties as assigned.
Employee must have his/her own transportation to carry out job duties and responsibilities. The vehicle must meet legal requirements (insured, registered, etc.) and the employee must have a valid driver's license.
