Responsible for providing case management to formerly incarcerated individuals living in The Academy or community; managing a caseload of approximately 20-25 individuals.
Essential Duties and Responsibilities:
· Provide intensive, focused casework services and direct therapeutic services for criminal justice involved residents who are homeless, and with a history of substance abuse and mental health;
· Assess the needs of the residents and arrange for services to meet those needs including substance and/or alcohol abuse services; mental health services, housing assistance, job training, educational services, basic support needs;
· Develop person centered service plans and quarterly reviews with residents;
· Accompany residents to appointments as needed;
· Advocate for residents to obtain full range of needed services and ensure coordination of these services;
· Facilitate group meetings with residents;
· Conduct one-on-one sessions with the residents to conduct ongoing assessment of resident needs and to monitor progress;
· Conduct monthly room visits/home visits or as needed;
· Participate in clinical staff meetings and treatment planning conferences;
· Prepare and maintain accurate and complete progress notes, records, and reports in accordance with policies of agency, funding and regulatory bodies;
· Maintain positive working relationship with staff, funding sources, community groups and other external service providers involved with the residents;
· Provide emergency on-call coverage during weekends, weekday evenings and holidays; and
· Other responsibilities as needed.
· High school diploma or GED/HSE required. Associates or Bachelor’s degree in Human Services field and/or CASAC preferred;
· At least one year of counseling and case management experience serving a similar client population;
· Ability to use Microsoft office and databases;
· Strong written and verbal communication skills;
· Bi-lingual English/Spanish a plus;
· Strong organizational and time management skills;
· Strong team building skills;
· Able to work independently and demonstrate good judgement;
· Ability to work flexible hours to cover others schedules as needed; and
· Driver’s License and good driving record a plus.
Travel within the five boroughs to conduct home visits and as needed to Long Island City office for meetings and trainings.
