The Supervisor is responsible for overseeing that all activities occurring within the case are focused on the youth achieving a favorable re-entry to the community and family reunification. Responsible Intake and for managing the Case Planners, After Care Worker and Navigator, the Supervisor assures that these staff integrate and apply Solutions Based Casework model and evidence-based models associated with GrahamUP into case practice activities with a focus on achieving permanency as safely and expeditiously as possible. Assures that the Case Planner develops and sustains working relationships with the youth family through interviews in the birth & foster homes and in the community to promote reunification and permanency ;Instruct and coaches Case Planners in the application of SBC and evidence based models associated with GrahamUP so that it is reflected in the daily casework activities conducted with the child, parents and caregivers; Incorporates language and terminology that is consistent with SBC and evidence-based; Assures that in conjunction with the birth parent, the case Planner completes a Family Genogram and Family Agreement (inclusive of Action Plans) using Solutions Based Casework practice. Amends the Family Agreement as necessary to assure that action plans are realistic and appropriate for the child/family needs; Assures that referrals to community service providers for birth parents are completed timely to encourage behavior change and promote family reunification; Promotes services and activities that strengthen youth development and preparedness for adulthood by assuring the proper incorporation of Permanency Pact in creation of an action plan.; Provides unit and individual supervision to case planners addressing case-related issues and themes, case progress and adherence to the principles of SBC and evidence-based models; Conducts performance evaluation for each assigned worker on an annual basis at minimum.; Celebrates success, provides corrective and progressive discipline as indicated; Responds to client emergencies within or outside work hours. Other duties as assigned.
Masters of Social Work or equivalent human services graduate degree; Must possess a valid state driver’s license
An applicant for employment will not be hired if his or her driving record reflects any of the following infractions; some examples include but are not limited to:
• Suspended, revoked or otherwise invalid license
• Reckless , high risk or otherwise unsafe driving behavior within the past 3 years
• 6 or more points on license
IT IS A PLUS IF YOU:
Are Bilingual; Have knowledge of OCFS regulations and mandates; Previous experience in: community organizing, political campaigns and/or customer service ; Have prior experience with the Juvenile Justice or Prison systems.
