Position Description:

The Social Worker collaborates with an interdisciplinary health care team and other service providers in the clinic to offer psychosocial services. Clients who access these services might be individuals experiencing homelessness or who are marginally housed, multiply diagnosed with a high behavioral acuity; active substance users; refugees, political asylees, immigrants or undocumented individuals and/or recently released from incarceration.


Essential Functions:

·        Collaborate intensely with interdisciplinary health care team

·        Conduct assessments, evaluating the social, emotional, and physical needs of clients;

·        Provide case management for clients, service referral, linkage, coordination and advocacy services for clients

·        Educate and support clients to be better access different systems of care (medical, complementary therapy, mental health, substance use, etc.)

·        Conduct mental health and substance use therapy and counseling

·        Develop and renew clients’ individualized client treatment plans regularly, and

·        Coordinate with clients’ medical providers weekly

·        Reinforce treatment goals that clients develop with medical providers

·        Complete necessary documentation, UOS reports and treatment plans in a timely manner

·        Participate in weekly case conferences, monthly staff meetings and in-service trainings;

·        Perform other duties and tasks as assigned.


Minimum Qualifications

·        Masters in social work or related field, license preferred

·        Two years of experience providing counseling, advocacy, education and service coordination and/or outreach work to disenfranchised communities

·        Experience working with the LGBTQ community and knowledge of the communities specialized needs

·        Knowledge of HIV/AIDS, STIs, substance use, mental health, social service and probation system

·        Familiarity with HIV/AIDS issues in communities of color

·        Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques

·        Must be adaptive, flexible, and a team player

·        Strong verbal and written proficiency in English

Preferred Skills

·        License in clinical social work or equivalent

·        Spanish language skills
