Job Summary:  Sacred Heart Respite Case Manager will provide comprehensive assistance and support for residents coming from the streets, withdraw management programs or other unstable housing environments in the development of realistic goals and achievable objectives as it relates to housing, employment, recovery, vocational, and financial stability.  Utilizing a person first approach the case manager will discuss individual dreams, concerns, and fears to assist in addressing potential barriers in achieving their identified recovery plan goals while working to ensure a holistic approach that achieves stability in mind, body and spirit so that individuals can be well on the path to achieving their long-term goals.

Essential Duties and Responsibilities

  • Support the Program Manager and Peer Specialist to complete initial assessments, including a needs assessment to prioritize residents needed services, and if needed identify appropriate alternative housing placement.
  • Complete a needs assessment within the first 5 days to prioritize residents’ short term and long-term goals including identifying long term housing goals.
  • Assist residents in acquiring entitlements, benefits and other supports as outlined from the Needs Assessment.
  • Provide input and guidance to participants in the development of realistic goals and objectives for Recovery plans.
  • Connect residents to ancillary services to create and/or strengthen recovery capital, establish community supports and linkages to outside treatment, TCM/BHSI or other case management as well as, employment, vocational or educational services.
  • Ensure connection to comprehensive medical services such as SKWC or other medical providers based on resident choice.
  • Complete Housing referrals and application packages for resident's transitions to appropriate housing Programs within city including the COC network, Journey of HOPE, and Safe Havens as well as other desired housing options.
  • Enter necessary information into ETO, HMIS and other databases such as web focus.
  • Complete necessary reports and submit to Program Manager in a timely manner.
  • Encourage residents to participate and support various functions and opportunities at Project HOME
  • Work with the Peer Specialist in assisting residents to maintain a clean and orderly living environment. Identify therapeutic work tasks in the residence that build skills and strengthen self-esteem.
  • Address behaviors of residents that impede their personal growth and recovery. Provide feedback and support to assist them in utilizing pro-social skills and making positive life choices.
  • Attend team meetings, case consultations, and provide support and guidance to peer staff as needed.
  • Make any other necessary referrals to services as needed for residents.
  • Provide transportation in site vehicle for residents when necessary
  • Support team in monitor resident's medication intake and overall compliance as needed.

Minimum Qualifications

  • BA/BS degree with at least 2 years’ experience as a Case manager or a high school diploma or Equivalent  with three to five years case management experience in the social service system and the homeless provider network; working knowledge of the drug and alcohol treatment system, and/or the behavioral health system.
  • Excellent listening, communication, collaboration, and teamwork skills.
  • Good computer skills, preferably experience with electronic recordkeeping and familiarity with Outlook.
  • Driver’ license

Preferred Qualifications

  • Experience with various housing models such supportive housing, housing first and recovery residences.
  • Experience working with individuals with a diagnosis of substance use disorder and or a mental health disability.
