Strengthening Families Together Project (SFTP) Case Manager is a full-time, exempt position with benefits who will provide intensive, therapeutic case management services to families experiencing challenges with housing, financial security and/or mental health issues.
SFTP is based on the Five Preventive (Proactive) Factors for Strengthening Families (adopted from the Center for the Study of Social Policy’s Strengthening Families Project) framework: (1) Parental Resilience, (2) Social Connections, (3) Knowledge of Parenting and Child Development, (4) Concrete Support in Times of Need, (5) Social Emotional Competence of Children
Duties and Responsibilities:
- Conduct intake and baseline assessments
- Conduct benefits screening
- Conduct brief therapy and crisis intervention services to provide a bridge and linkage to longer term therapeutic supports if needed
- Assist families with completing and submitting social service benefit applications
- Individualized service plan (ISP) development
- Comprehensive information and referral services to housing, financial literacy, and mental health supports
- Tracking program participant service utilization and outcome data
- Making and tracking referrals (internal and external) for specialized services
- Conduct Parent Workshops
- Coordinate Financial Literacy Workshops
- Advocacy for families
- Assist with housing relocation, eviction prevention, housing search, landlord liaison activities, financial coaching, and housing counseling.
- Link to housing service providers and coaches
Qualifications:
- Master’s degree in social work, human services, or related fields
- Have case management experience and experience with brief therapy or task-centered counseling
- Must demonstrate a genuine passion for community organizing, community youth development, and working with populations often denied access to vital human services due to social or economic barriers
- Must be skilled and passionate about developing and implementing intensive case management systems and processes
- Must have excellent organizational, management, and interpersonal skills, as well as strong written and verbal communication skills and team building skills
- Must be able to utilize planning time effectively and consistently produce top quality results
- Must be an excellent problem-solver who can balance multiple tasks and responsibilities
- Must complete at least twenty hours of professional development training per year
- Must attend all required staff meetings and trainings
- Skilled or trained in motivational interviewing, a plus
- Ability to supervise graduate student interns in social work, counseling, or related fields, a plus
