Thresholds’ Emerging Adults Division is seeking master’s level clinicians to fill a Community Support Specialist role at our growing Chicago site. The Emerge program uses evidence-informed coordinated care approaches to support young adults struggling with serious mental health challenges. These services are aimed at changing the trajectory of mental health needs for young people and is part of a national innovative movement in supports for young adults. The multidisciplinary team provides specialized assessment, care coordination, individual therapy, medication management, vocational/educational support, & community outreach utilizing evidence informed approaches. The new Community Support Specialist will receive expert training in our treatment models, including the Transition to Independence (TIP) Model & play an integral role in the growth & development of our Emerging Adult programs.

The Community Support Specialist will be responsible for generating treatment plans, providing case management, community-based learning,, crisis intervention, therapeutic support, and care coordination. In addition, they will assist with facilitating activities and programs that will contribute to young adults’ overall development. The Community Support Specialist works within a team-oriented, strengths-based approach, ensuring collaboration within the team and with other providers to facilitate integration of services.

The successful candidate will have experience providing mental health services to young adults and possess a strong foundation in trauma informed care and motivational interviewing. This person should have good written and verbal communication skills and strong basic computer skills, which include the ability to document services in a clear and effective manner. They must function well independently and as part of a team. Must have a valid driver’s license and daily access to a reliable personal vehicle. Masters is required, LSW/LPC/LCSW/LCPC and Experience with DBT and Expressive Arts is a plus.
