As a Community Health Center focused on improving the lives of our patients, Harbor continually evaluates and improves upon processes throughout the organization. The Quality Improvement Analyst and Project Coordinator works with staff at all levels – front lines, leadership and support teams – to push forward quality projects that lead to better care and quality of life. This requires high levels of organization, acute observation skills, and the ability to relate to others in the pursuit of Harbor’s aspirational goals.
Sample projects include: developing processes and supports to help care teams identify and support patients at high risk, improving rates of screening tests for specific conditions (e.g., annual foot exams for patients with diabetes), etc.
•Collaborates with quality and applications support teams to improve measures, processes and overall patient care experience. In so doing, develops deep understanding of reports and technologies.
•Develops clear, engaging summaries and presentations for staff and leadership
Data and analysis duties:
•Analyzes processes to identify efficiencies and improvement opportunities.
•Researches best practices; conducts quantitative and qualitative analysis, works with technology team to identify system possibilities and/or constraints.
•Facilitates and/or completes investigation of new quality measures including data validation, workflow investigation and data mapping. Identifies and captures baseline data for improvement projects.
•Works with applications and technology teams to identify system possibilities and/or constraints.
Planning and management of quality improvement projects:
•Works with individuals and groups by running brainstorming sessions, conducting interviews, arranging meetings/working sessions and coordinating tasks.
•Engages staff at all levels to participate in large-scale quality improvement efforts.
•Develops and iterates on deliverables, process, and other project outputs to optimize solutions and new workflows.
•Supports administrative project needs including note-taking, planning, scheduling meetings and following up on action items.
•Monitors progress and success of improvement projects. Ensures adoption using data and experiential information from staff.
Required Education, Experience and Skills:
•2 plus years of relevant work experience in the fields of health care, social services, government or the consulting field
•Bachelor’s degree or two additional years of health care experience
•Master’s degree and/or project management training preferred
•Familiarity with Quality Improvement models such as that used by the Institute for Healthcare Improvement (IHI), Lean Technology, Plan-Do-Check-Act model, and other process improvement theories and practice
•High level of skill in presenting and developing polished presentation materials
•Basic to Intermediate Excel/spreadsheet skills required (e.g., formatting, simple charts, pivot tables)
•Excellent critical thinking – able to break down problems to diagnose accurately and comprehensively develop solutions
•Experienced in project management and group facilitation, including: project management tools, tracking tasks, driving accountability, running effective meetings
•Excellent observation skills – able to quietly take in information in varied settings
•Excellent communication and emotional intelligence
•Interested in and attentive to details and nuance; equally able to manage ambiguity and abstraction
•Values listening and participation in the process of improvement
•Independent learner – tests, researches and gleans new skills on own
