Information and Assistance Specialist

The Healthcare Consortium is seeking a full-time (35 hrs/wk) Information and Assistance Specialist to work within the NYConnects Program, both in our Hudson office and at satellite locations throughout Columbia County.

The role of the Information and Assistance Specialist is to help people, their family members, and other caregivers learn about aging and disability resources that are available in and around Columbia County. By telephone, on-line, or in-person, the Information and Assistance Specialist helps link people to long term services and supports, coordinates with other agencies to guide people through eligibility processes, provides seamless linkages for any necessary assessment that may be needed, helps with application and enrollment in public assistance benefits, and conducts program outreach. 

The minimum requirements for this position are a high school diploma, GED or TASC, a valid NYS drivers’ license, and a reliable vehicle for travel to worksites, outreach activities, and the occasional home visit. 

Since any new person in this role will undergo extensive training through reading, on-line training modules, and coaching, we will consider applicants with a variety of work experiences (e.g. hospitality). However, the person who fills this role MUST be motivated to help others, be patient and courteous, and have an instinct for working a problem until it is solved. A knowledge of Columbia County and its public and private providers of health and human services is helpful, but can also be learned. 

The Healthcare Consortium’s offices, where this individual will spend most of his/her/their time, are located in an ADA-compliant facility and observe COVID-related workplace health and safety protocols. The candidate must also be able to work at alternate worksites, including the home, and on the rare evening or weekend.

The wage for this non-exempt position is $20/hour with a rich benefit package.
