Lantern Community Services is a supportive housing organization that champions the independence and well-being of New Yorkers who are impacted by or threatened with homelessness. Lantern provides comprehensive services and programs designed to support clients so they can stay healthy and housed. Our 15 buildings, with almost 1,400 units, house a range of single adults to families, with sites located in the Bronx, Brooklyn, and Upper Manhattan. Lantern currently has a staff of approximately 120 full-time employees and services a client base of nearly 1,200 and an additional 800 community residents.
Lantern Community Services’ teams of experienced social workers and program specialists deliver programming directly within each apartment building to these high-need populations, resulting in excellent rates of engagement with our clients. Programs with proven results in health, employment, education, and life skills are delivered inside our residences and tailored to each person’s needs. We provide Lantern clients – many of whom live with mental illness, addiction, disability, HIV or other chronic illnesses – with the tools they need to integrate successfully into the greater community with dignity and independence.
Location and Work Schedule:
This is a full-time Monday through Friday position at Hunterfly Trace, located in Brownsville, Brooklyn, which requires a flexible schedule.
We are seeking a dynamic and engaging Case Manager who has experience working with chronically homeless, mentally ill, single adults (between the ages of 23 and 65) population in a supportive housing environment. The Case Manager will be expected to assist this population in stabilizing their lives, maintaining their housing stability and attaining their physical, mental health, work, community, educational and social goals. The ideal candidate will have solid clinical knowledge as well as experience working with the chronically homeless population that has been impacted by mental illness, substance abuse, legal and medical issues. Additionally, the Case Manager will be passionate about working with single adults; is a proactive and creative problem solver; and works well both as a part of a team and independently. The Case Manager works closely with an interdisciplinary team to build and maintain a safe program culture that emphasizes respect for all community members and ensures all clients progress forward in their goals. The Case Manager reports to the Program Director.
· Develop individualized service plans that will ensure a seamless system of services that are culturally sensitive and client-centered; identify short and long-term treatment goals and helps residents achieve them; and include referrals and are collaborative with other staff, community providers and caregivers.
- Create and maintain thorough and comprehensive case record of resident services and progress, including progress notes, service plans and other relevant documentation.
- Address a wide-range of resident needs including benefits, money management, medication management, apartment management, life skills and health and mental health issues.
- Serve as a liaison with various agencies, health care providers, mental health providers, and employment training providers to secure services. Accompany clients to visits as needed.
- Work collaboratively with an interdisciplinary team to create and maintain a safe and stable program culture built on a foundation of respect for all members of the community.
- Develop groups that are appropriate for the residents’ needs while also conducting individual and group counseling services.
- Attend regularly scheduled case conferences, weekly staff and clinical meetings, supervision, trainings and special agency staff meetings.
- Perform apartment/room inspections as needed.
- Perform other job-related functions, as assigned.
· BA/BS required, preferably in a related field such as social work, sociology or psychology; MSW is preferred.
- Bi-lingual Spanish/English preferred but not required.
- At least 2 years of experience working with single adults impacted by poverty, chronic homelessness, mental illness, substance abuse and medical issues.
- Ability to work effectively independently as well as a member of an interdisciplinary team.
- Highly organized and able to manage a diverse case load of clients with diverse needs.
- Able to work well under pressure including in crisis situations and able to be flexible with work schedule when necessary.
- Track record of proactive, strength-based work with individuals and families.
- Experience with ETO, or other electronic case management systems.
· Experience working with evidence based models, specifically Motivational Interviewing, preferred.
