    Description

    As the Case Manager, you will work with a team of staff to facilitate creating a safe, healing environment for guests residing at House of Hope, our homeless shelter in North Bend, WA. Part of our team provides advocacy-based counseling, information, referral, and support services to program guests.

    Responsibilities

    • Manage a caseload of individual residents.
    • Meet residents weekly and maintain all electronic and paper client files.
    • Provide crisis intervention information, chemical dependency case management, trauma informed therapy and referral help.
    • Communicate with all staff.
    • Help coordinate and facilitate support groups, therapy classes, curriculum, and classes.
    • Develop and maintain ongoing guest files.
    • Help maintain a positive and pleasant atmosphere at the facility.
    • Attend and participate in all Mission, Division, and Department meetings and training, as required.
    • Contribute on-going support through our Wrap-Around Care Program

    Qualifications

    • An Associate Degree is required. A Bachelor's Degree is preferred. A combination of education and four years of relatable experience may be substituted.
    • Three years of experience working in a social services or rescue mission setting desired. One of those years must be in a Case Manager role.
    • Proficient in Microsoft Office Suite.

    Work Environment While performing the duties of this job, the employee is frequently required to sit for periods of time, frequent use of computers, keyboarding, and talking on the telephone. May lift up to 20 pounds and walks, uses stairways to climb to multiple floors within the facility.
