We do this by delivering impactful services with measurable outcomes to children, youth, adults, and the elderly. As a civic anchor, we carry out this mission within the context of building a sustainable community for all people through the arts, environmental advocacy, and urban planning. 

Established in 1994, St Nicks Alliance’s Youth and Education department strives to transform the lives of more than 6,000 North Brooklyn youth, ages 2-24, each year. Through a wide-range of programs, youth are empowered to successfully advance through the stages of education and into young adulthood with the core competencies to thrive, both emotionally and financially. In partnership with schools, families and community, St. Nicks Alliance’s holistic approach to transformation combines youth development, academic enrichment, health and wellness, arts learning and post-secondary readiness.  

Job Summary

SNA’s Youth and Education department seeks a full-time Case Manager who will work with a cohort of students as they transition into high school. Responsibilities include meeting with students individually on a weekly, bi-weekly, monthly, or more frequent basis if additional support is needed, to develop assessments and goal setting. Emphasis is placed on supporting and advocating for students' needs in order to develop a positive youth culture that fosters unity and a stronger sense of self. Our aim is to create a strong relationship with the schools we partner with in order to ensure the best support possible to all throughout the school community.

 

Responsibilities

  • Mentor youth, to help them set and achieve goals for education, employment, and self-efficacy as well as manage and negotiate obstacles to success.
  • Manage and facilitate social and emotional learning groups with youth  
  • Develop leadership curriculum and lesson plans to facilitate groups for youth on life and leadership skills.
  • Monitor students' attendance at school and obtain necessary school reports (report cards, attendance, transcripts, etc.).
  • Maintain communication with parents/legal guardians as needed 
  • Maintain updated progress notes for individual caseload and groups on a weekly basis.
  • Input all relevant data into ETO (Efforts to Outcomes) database on a weekly basis.
  • Collaboration with all DOE school personnel 
  • Make appropriate referrals for youth and/or their families on and off school campus (social worker, medical, housing, substance abuse, CBO’s or other health services as needed)

 Requirements:

Bachelor’s Degree in a related social science field and demonstrated experience working with youth. Must have a strong understanding of the developmental needs of young adults/at-risk youth, with sensitivity to ethnic and cultural diversity, and knowledge of and commitment to working from a strength-based, youth-development perspective. Applicants must have excellent written and verbal communication skills. *Bi-lingual preferred (Spanish)*
