The Program Director will provide both oversight of outreach and critical direct client services to St. Mary’s Outreach Center, including benefits assistance, referral services, service coordination, and supply distribution. The Program Director will work directly with our partner organization Action in Maturity (AIM) in organizing programmatic activities (educational and social activities) plus allowing for seamless delivery of comprehensive services. AIM is a senior center without walls, based in Hampden and providing complementary services to older adults in our shared service area.

 

Key Responsibilities


1.     Maintain active caseload of approximately 50 clients per month and perform the following tasks (>70% of role):

a.     Conduct client-centered intakes, assess strengths and needs, and develop, monitor, and evaluate individual action plans and progress. Coordinate care as needed.

b.    Perform benefits screening. Assist clients in completing and submitting applications, and perform advocacy on client’s behalf, if necessary, for client to obtain benefit.

c.     Manage appropriate referrals to other agencies by making warm referrals, assisting client to complete necessary forms and processes, and removing barriers to successfully accessing the referral.

d.    Develop and facilitate the relationship between different service providers in order to maximize benefit for the client. Communicate with a wide range of social, governmental, and legal agencies to ensure continuity and coordination of care for client.

e.     Maintain documentation, client files, and statistics as required by agency policies and funding sources.

2.     Coordinate the development and implementation of educational and social activities (20% of role).

3.     The remaining ~10% of the role will support the following activities:

a.     Support the supply distribution efforts of the organization by contacting clients, scheduling deliveries and distributions, and coordinating with relevant agencies and vendors.

b.    Provide task-management for volunteers supporting the organization’s case management efforts and supply distributions. 

c.     As needed, attend community meetings and support organization in establishing and maintaining a working network of community agencies as referral sources for clients.

d.    Provide support for the development of grants, contracts, and related reports. This may include program development and data reports. .




Qualifications & Competencies


Required

·       Bachelor’s degree in social work, human services, or related field

·       Minimum 5 years of experience with service delivery to older adult clients and managing a staff.

·       Belief in a strengths-based approach to human services

·       Excellent coordination and organizational skills and meticulous attention to detail

·       Ability to work independently and collaboratively with colleagues

·       Ability to work well with older adult clients, especially when they are in crisis

·       Ability to provide respectful, courteous, professional service to persons of various social, cultural, economic and educational backgrounds

·       Ability to move objects up to 30 pounds

·       Excellent command of the English language and strong communication (oral and written) and interpersonal skills

·       Demonstrated skill in using Microsoft Office Suite

·       Must be able to pass State of Maryland and U.S. Federal background and criminal screen

·       Ability to move freely throughout the community and to complete home visits in a variety of settings, to include climbing stairs

 

 

Employment Details


The Program Director will work 37 1/2 hours a week, typically between the hours of 8:30 and 5 PM. This a non-exempt, hourly position. Compensation is $22-$24 per hour.


The Program Director will report to the Interim Director of St. Mary’s Outreach Center.
