The Health Director at La Colaborativa trains, monitors, motivates and supervises health promoter activities and engages in partnerships for strategic expansion of activities. Health promoters work with the community to mitigate social determinants of health, educate on health risks, including disease distribution of COVID-19, through door to door and street outreach, needs assessment with Chelsea residents, referral to local resources, including social protection services and COVID testing and vaccine clinics. The health director works with the health supervisor to plan activities, strategies and monitor and report outputs from activities. The health director works from a gender-based equity lens to improve the lives and wellbeing of families disproportionately impacted by social determinants of health including the COVID-19 pandemic, systemic racism, and economic inequities in Massachusetts.

Health Director Job Responsibilities:

Strategic planning

  • Work with the COO and Director of Development to plan departmental strategies for the year.
  • Participate in grand proposal planning and writing.
  • Maintain transparent and constant relationships with donors, reporting on time, participating in meetings and evaluations.
  • Work with health promoters to plan weekly activities, monitor the impact of previous activities and adjust accordingly.
  • Use current public health data to inform weekly, monthly and annual planning.

 Monitoring and Evaluation

  • Collect daily and weekly output data on activities.
  • Ensure outputs correspond to grand proposal outputs.
  • Articulate outcomes from project outputs and report accordingly.
  • Present project results to leadership, partners and donors in a timely way.

Partnerships

  • Participate in weekly and monthly calls related to health activities.
  • Maintaining positive, transparent and constructive relationships with local and statewide partners.
  • Seek new partnerships for improved outreach and expansion of activities.
  • Maintain professional codes of conduct, including confidentiality in all professional relationships.

Supervising Health Promoters

  • Work with the health coordinator to plan outreach and health promotion activities of the Health Promoters Team including; outreach schedule, public health messaging, presence and deployment to health fairs, mobile van, food pantries and other public gatherings.
  • Manage, train and supervise the Health Promoter team through activities such as recruitment, capacity-building, briefing/debriefing and evaluation of staff.
  • Supervise the Health Coordinator in planning the weekly schedule and outreach target areas.
  • Create a motivating and continuous learning atmosphere with the Health Promoters Team through regular structured supervisions and on the job training.
  • Perform trimester evaluations of staff.
  • Ensure that Health Promoter Teams have materials (fliers, masks kits, outreach information etc.), and equipment (iPads during survey data collection) needed to perform duty.
  • Train and supervise on data collection methods during survey roll outs (vaccine hesitancy, needs assessment, etc.)
  • Conduct regular team meetings (daily/weekly). 

Planning and Implementation

  • Implement and supervise public health education and outreach campaigns to increase local awareness and understanding of current public health concerns and crises. These include (but are not limited to) COVID-19 risks, prevention strategies, prevalence, access to testing, and access to vaccines in close collaboration with the Health Equity Director.
  • Ensure outreach and education strategies are designed from a foundation of improving and understanding the social determinants of health that threaten the health and wellbeing of Chelsea residents and increase negative health outcomes, acute and chronic disease.
  • Define and update the Health Promotion strategy and related activities through a monthly plan (objectives, target population, campaigns, messages, activities, etc.).
  • Analyze and understand trends and changes local behavior surrounding health, beliefs and/or practices to adapt health promotion activities to current needs.
  • Assess and adapt information and education materials according to the needs of the project and current health threats identified within the community.
  • Promote La Colaborativa services according to internal principles, protocols and standards with the aim of increasing the knowledge and empowering Chelsea residents in relation to their health.
  • Liaise with other relevant actors and partners to enhance campaigns and support the dissemination of health information.
  • Be familiar with protocols for health promotion and prevention, including COVID-19, and ensure their application.
  • Identify constraints, difficulties and challenges to the health promotion strategy and work with the Health Coordinator and the health promoters to overcome these.
  • Model excellent health behavior including proper mask wearing, social distancing, and other COVID protections.

Administration and Organizational Participation

  • Ensure weekly outputs and monthly outcomes are reported in a centralized database.
  • Conduct community-based analysis to improve outreach and education.
  • Coordinate with other directors at La Colaborativa to stay informed of available services and resources and refer families appropriately.
  • Maintains positive and constructive relationships with community leaders, organizations and health institutions.
  • Enhances organization reputation by accepting challenges, maintaining positive relationships with the community and exploring opportunities to add value to health promotion.

Education, and Experience Requirements

  • Bachelor's Degree in health sciences or promotion
  • Master's Degree in public health, social work or related field preferred.
  • 5+ years experience in managing teams, implementing projects and leading public health activities.
  • Demonstrated experience leading teams.
  •  Demonstrated experience managing grant funded initiatives.
