The San Francisco 49ers Academy has two Caseworkers positions open at its Middle and High School Programs.

THE ORGANIZATION

Located in East Palo Alto, the San Francisco 49ers Academy (49ers Academy) is a nonprofit organization integrated into the school day to meet students’ individual needs and promote a path to academic success by:

  1. Building partnerships with parents, teachers, school district leaders, and community service agencies to ensure students’ holistic needs are met. 
  2. Providing individualized wraparound support and case management through mentorship, family empowerment, academic success, enrichment opportunities, community engagement, and social-emotional learning

Position Overview

The 49ers Academy has two Caseworkers positions open at its Middle and High School Programs. The Caseworker position is critical for the 49ers Academy at this stage of our growth. Caseworkers will mentor students individually regarding academic support, school completion, behaviors, attendance, and other academic/social/emotional barriers that may interfere with school success and promotion/graduation requirements. The Caseworker will also model and utilize best practices for family engagement to increase parent/family involvement and connectivity and to foster a broad sense of school community.

Middle School Program

This position will primarily work with Cesar Chavez Ravenswood Middle School students to help ensure a smooth transition throughout their middle school experience. The 49ers Academy’s middle school program provides a personalized community of support for students in grades 6-8 to promote academic skill-building and social-emotional learning that builds a foundation for a positive school experience. This position is supervised by the Middle School Program Manager and collaborates with the 49ers Academy Programs team.

High School Program

This position will primarily work with Menlo-Atherton High School students to ensure a smooth transition throughout their high school experience. Building on strong relationships developed during middle school, the 49ers Academy provides a safe space for East Palo Alto students as they navigate Menlo-Atherton High School. Through academic support and needs-based services, the 49ers Academy High School program aims to keep students on track to graduate and pursue post-secondary endeavors. This position is supervised by the High School Program Manager and collaborates with the 49ers Academy Programs team.

Primary Duties and Responsibilities include but are not limited to the following:

Case Management and Student Support

  • Develop a comprehensive Individual Success Plan (ISP) that captures the student’s needs, specific interventions and services, frequency of interaction, monitoring and adjustment of services, and evaluating and reporting progress and outcomes.
  • Provide 1:1 intensive individualized mentorship and support to address needs, monitor effectiveness of provided support, and adjust accordingly.
  • As assigned, coordinate and support activities, focusing on basic needs, enrichment, and school culture and climate support.
  • Maintain confidentiality regarding students, personnel, and all internal organizational information.
  • When appropriate, attend all critical educational meetings to advocate for students’ needs while initiating change at a system level.                 

School & Family Support

  • Collaborate with school administrators, faculty, and staff to meet the plan’s parameters and provide successful school success intervention strategies.
  • Ensure regular communication between school staff and families, which includes sharing ISP goals and progress.
  • Acts as a liaison between families and school personnel and mitigates problematic behavioral or academic issues and barriers to school success.
  • Meet with families, middle school officials, or faculty and staff of incoming ninth-grade students to formulate plans toward a smooth transition to Menlo-Atherton High School.
  • In collaboration with the Community Resources Manager, connect and refer identified students and their families with external programs, social agencies, business partners as appropriate and as established in the intervention plan; and monitor the effectiveness of such programs/services.

Data Management 

  • Ensure that all case management and other student support activities are documented. 
  • Ensure that student case files and digital records (in assigned Data Management System) are updated weekly, i.e., notes and service changes. 

Additional Job Functions

  • Attend weekly caseworker meetings, training (as requested or assigned), departmental meetings, and monthly staff meetings.
  • Represent and promote the 49ers Academy at conferences, speaking engagements, related events, and occasional media opportunities as necessary. 
  • Performs other related work as required and assigned.

Position Structure

  • Typical schedule Monday-Friday 8:30 am-5:00 pm - Schedule will vary according to programmatic needs, events, and activities.
  • May attend and participate in events as required during out-of-school hours.

  • Ability to maintain a calm, professional demeanor in the face of competing demands and external pressures
  • Preferred but not required, Bilingual capability (Spanish)
  • Ability to manage details and work independently
  • Ability to work effectively in changing the work environment
  • Maturity, humility, strong work ethic, sense of humor, and roll-up-my-sleeves attitude


MINIMUM SKILLS & QUALIFICATIONS

  • Bachelor’s degree in education, social work, child development, or a related field required with at least 1-3 years of demonstrated relevant experience working in a community-based nonprofit or educational institution, working with youth, families, and local community members, as well as diverse populations; OR Graduation from a two -year college or university with an Associates Degree and at least five years’ experience.
  • Excellent writing, public speaking, and interpersonal skills 
  • Attentive to detail, highly organized, and capable of managing multiple projects and meeting deadlines.
  • Possess strong computer skills, particularly with Microsoft Word, Excel, and PowerPoint.

PHYSICAL DEMANDS

This is primarily a sedentary role; however, some physical demands listed below are required to successfully perform this job’s essential functions. 

  • Typing, writing, reading, hearing, and speaking
  • Use of hands and fingers to operate office equipment
  • Frequent sitting
  • Standing, walking, kneeling
  • Some lifting and carrying up to 30lbs
