    Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers.     Program Description: The Young Adult Assertive Community Treatment Team is a unique and nontraditional licensed outpatient psychiatric clinic for individuals, 18-25 years old with Serious Mental Illness (SMI) who have demonstrated a high service need with multiple hospitalizations, emergency room visits and difficulty maintaining stability in the community. ACT is a nationally recognized model and an evidence-based practice. The ACT Team serves all of Manhattan and occasionally the outer boroughs.

    Purpose of Position: The Team Leader provides administrative and clinical support for the ACT team.

    Salary Range: $68,640-$75,000

    Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

    Roles, Responsibilities, and Essential Duties

    1. Provide case management services
      1. Carry a small caseload.
      2. Perform client assessments and formulate treatment goals
      3. Facilitate client participation in entitlement programs, psychiatric services, medication management, rehabilitation and other activities
      4. Refer clients to community health, substance use psychiatric and other services
      5. Accompany clients to appointments as needed
      6. Communicate regularly with clients’ families about their needs and progress
      7. Serve as liaison to other service providers
      8. Maintain up-to-date client case records according to program guidelines
      9. Visit clients in residential facilities, on the street and in other locations and advocate with & on the client's behalf.
      10. Rotate on-call service outside of work hours
    2. Management, Supervision
      1. Provide clinical supervision to staff and interns; evaluate their work and discuss progress of their clients, teach invention and advocacy skills
      2. Assist in recruitment of ACT staff members
      3. Assist with program administration, including coordinating staff schedules and overseeing adherence of staff training requirements
      4. Assist in ensuring staff is adequately trained according to OMH and ACT standards
      5. Perform Program Director’s duties in his/her absence
    3. Participate as an ACT team member
      1. Attend staff meetings, case conferences, and training programs to upgrade skills and knowledge.
      2. Attend supervision meetings to review client progress and case management strategies
      3. Work collaboratively with ACT team staff in a team case management model
    4. Policy and Compliance
      1. Assist in the development and adherence of Policies and Procedures in accordance with Goddard Riverside, New York State Office of Mental Health and Department of Health and Mental Hygiene
    5. Other duties as required

    Qualifications/Educational Requirements

    • Licensed Master Social worker (LMSW) required
    • SIFI (supervisor in field instruction) strongly preferred

    Skills, Knowledge and Abilities

    • Experience working with the seriously and persistently mentally ill population required
    • Experience working with Young Adult population preferred
      • ACT Team experience preferred
      • Ability to speak Spanish, French, Creole, American Sign Language or other languages is a plus

    Computer Skills: To Perform this job successfully, an individual should be

    • Able to work in the Microsoft Office Suite, should especially be proficient in Word and Outlook
    • Able to use or learn to use AWARDS database
    • Able to use or learn other electronic databases, programs, and funder-related software as needed
