Licensed Home Care Agency is looking for an Assistant Program Director
Reports directly to the Program Director of New York Foundation for Senior Citizens Home Attendant Services and responsible for the following:
Assists in ensuring that all Quality Improvement monitoring activities are conducted and documented including, but not limited to, file reviews, unannounced telephone calls and home visits, risk management, home care worker training, etc.
Assists in ensuring that all changes in authorizations are transmitted to the Personnel Specialists and the Personnel Specialist Supervisor ensures that the correct number of hours are provided.
Assists in monitoring and ensures that the Personnel Specialist Supervisor also monitors the Schedule Verification Report and ensures that the Personnel Specialist Supervisor ensures that the Personnel Specialists obtain all required timesheets on time to ensure timely filing of all bills.
Assists in monitoring a sampling of 20 client files and progress notes per month to ensure that they are continually up-to-date and complete and maintained in all areas of contract compliance including, but not limited to, progress notes, annual medical examinations, authorizations, reauthorizations, etc.
Assists in monitoring computer output to ensure that all Home Attendant files and progress notes are up-to-date, complete and in continuous contract compliance including, but not limited to, in-service trainings, nurse’s visit reports, annual medical examinations, evaluations, etc.
Assists in reviewing all complaints and investigations and ensures that appropriate clinical and personnel actions are implemented.
Assists in ensuring that the Personnel Specialist Supervisor distributes the Travel Time Report to the Personnel Specialists and distributes the calculated travel time to the Assistant Director of Administration.
Assists in preparing and submitting contract compliance reports including Bureau of Labor Services Report, Home Attendant Vendor Agency Renewal Application, HRA Office of Program Contract Administration Cross Check Questionnaires, census reports and HRA Survey reports, etc.
Assists in ensuring that the Director of Patient Services and Personnel Specialist Supervisor logs complaints in client complaint log.
