POSITION SUMMARY: The Case Manager is responsible to provide HIV counseling and testing on and off site; coordination of care for identified needs, establish professional linkages with identified HIV/AIDS and health and social support services providers in the targeted project communities.

RESPONSIBILITIES:

1. Coordinate and link clients to services like treatment adherence support, Housing, entitlements, risk reduction education and health insurance

2. Coordinate and implement educational seminars on HIV/ AIDS/ STD prevention and availability of services.

3. Complete risk assessment  

4. Provides coaching and other service modalities to keep clients engaged

5. Assist patients at risk for HIV/ AIDS to access various community health, nutrition and social support services. 

6. Implement appropriate follow-up for client referrals to community health, substance abuse, mental health, nutrition and social support services.

7. Participate in staff meetings and staff development trainings.

8. Perform special projects and any other tasks as assigned 

MINIMUM QUALIFICATIONS:

1. Bachelor’s Degree and 2 years of experience working with the HIV population.

2. Two years as a full-time case management or comparable experience.

3. Good organizational skills.

4. Some understanding of medical terminology.

5. Ability to work with difference disciplines within the healthcare industry.
