Job Summary Women of Change is a safe haven residence that serves women who are living with mental illness and have experienced chronic street homelessness. The site seeks to address barriers to housing by working with residents towards holistic wellness.

Essential Duties and Responsibilities  

  • Provide case management services to assigned residents (10-15) including but not limited to identifying needs and establish goals and objectives using a person-centered approach. Assist residents with learning or improving skills to complete activities of daily living (scheduling and attending appointments, attending programs, recreational activities, personal hygiene, laundry, budgeting, cleanliness of personal space and adherence to house rules). Assist with referrals to community-based services, housing applications and income/mainstream benefits applications. 
  • Monitor medication as scheduled and maintain up to date medication charts and boxes for assigned residents. 
  • Recognize and manage crisis situations effectively. Utilize de-escalation skills; create an atmosphere of safety and accountability. Follow up as necessary with mental health, medical and/or treatment providers to connect residents to appropriate levels of care. 
  • Complete documentation as required including electronic record keeping system, resident files and completion of reports as required. 
  • Attend and participate in team meetings, case reviews and concurrent review meetings as scheduled along with supervision and other program or agency meetings as assigned. 
  • Demonstrate ability to work independently in the community by managing time and daily schedule, making decisions with residents in the field and acting on them as needed, adjusting schedule based on emergencies and/or priorities, meeting daily and monthly productivity requirements. 
  • Provide on-call coverage as scheduled. Must be able to work at least one weekday evening until 7:00 pm.


Minimum Qualifications  

  • Bachelor’s Degree with a minimum of 3 years case management experience 
  • Experience with individuals experiencing homelessness, mental illness and substance use. 
  • Strong organizational, interpersonal and written communication skills 
  • Good computer skills - proficient in Microsoft outlook, word, excel and electronic record keeping 
  • Ability to listen and respond with empathy, compassion and hope. Ability to implement person centered goal and recovery plans. 


Preferred Qualifications 

  • Master’s Degree in Social Work, Counseling or related field 
  • Residential and/or outreach experience 
  • Experience with Trauma Informed Practice, Motivational Interviewing, Stages of Change or other evidence-based practices. 
  • Familiarity with Philadelphia mental health, housing and/or substance treatment providers.
