Assistant Director of Care Coordination (Health Care Integration Centralized Costs)
Schedule: Monday - Friday, No weekends, No On-call
Salary: $80,000-$85,000
Responsibilities:
The Assistant Director partners with the Director to provide oversight of Behavioral Health Care Coordination as implemented through CP Care Teams in the Greater Boston, Southern and Northern Regions. This role directly supervises Team Leaders of a cluster of Care Teams in the provision of intensive care coordination and clinical care management for Mass Health Members with complex medical and behavioral health needs who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. Therein, the Assistant Director is responsible for hiring, training, supervising and evaluating Team Leaders. The role is responsible for the development and implementation of care team policies, procedures and work flows so as to ensure that each Care Team is meeting quality performance metrics and Enrollee satisfaction. In partnership with the Program Director, the Assistant Director collaborates with the Executive Director of CCP in developing and managing ongoing relationships with Enrollee’s ACO/MCO plans to minimize duplicative efforts, promote integrated care, ensure quality and continuity of care, and support the values of person centered planning, Community First and SAMHSA Recovery Principles.
