Summary: Under the supervision of the Resource Navigator Supervisor, performs remote work in support of COVID-19 contact tracing, with alternative work schedules and/or compressed work weeks (the program is live 9 am to 9 pm daily, including holidays and weekends schedules). Conducts interviews with individuals diagnosed with COVID-19 (“Cases”) and the individuals that they came into contact with (“Contacts”) around resource needs using a trauma-informed, culturally respectful approach that builds trust and facilitates the free sharing of information. Connects Cases and Contacts with identified resources. Elevates complex cases to Resource Navigator Supervisors. Follows all scripts, policies and procedures provided through training and complies with appropriate handling of confidential information related to case interviews and contacts. All work is conducted in a confidential manner. This is a full-time temporary position, initially through the end of 2020 with the possibility of an extension.

Responsibilities:

·        Conduct telephone calls with clients consisting of persons diagnosed with COVID-19 and those identified as close contacts to assess resource needs for self-isolation and quarantine.

·        Identify common needs amongst clients and search for local solutions. These could include access to food, mental health services, legal or financial counseling, and other community programs helping patients in need and/or people in isolation or quarantine.

·        Follow approved scripts and protocols, provide clients with vetted information about these local community resources, including those available at NMIC.

·        Connect clients with needed resources through various platforms.

·        Communicate with clients in a professional and empathetic manner and protect and maintain individuals’ privacy and confidentiality.

·        Effectively communicate and demonstrate the System’s acknowledgment and awareness of diverse health- related needs and concerns to patient, family members, caregivers, staff and the community.

·        Collect and record information obtained from the resource interview in requisite data systems in accordance with standard operating procedures.

·        Check records for accuracy of information and for conformity with established policies and procedures.

·        Manage assigned caseload and ensure prompt and accurate data entry and follow-up with clients and escalate cases to Resource Navigator Supervisor, as appropriate.

·        Maintain daily communication with assigned Resource Navigator Supervisor, including recognizing, documenting, and alerting the supervisor to trends in customer calls.

·        Complete all assigned paperwork and submit documents (e.g. Daily Logs) in a timely fashion; daily logs are to be submitted at end of each day.

Additional related responsibilities may be assigned.  

Requirements:

·        Two years of experience in social or human services or equivalent coursework (social work, human services, community/public health, sociology, psychology, mental health counseling, or related field)

·        New York City Resident preferred and familiarity with New York City neighborhoods, communities, and boroughs required.

·        Candidates of highly impacted communities are strongly encouraged to apply.

·        Candidates preferred to have experience working with community resources and social services in different parts of New York City that can inform connecting patients with COVID and their families to local resources (Please indicate what location in your cover letter).

·        Excellent interpersonal skills and ability to interact professionally, empathetically, and non-judgmentally with people from diverse cultural, racial, ethnic, gender, and socioeconomic backgrounds during a time of crisis and distress.

·        Excellent organizational and communication skills.

·        Ability to handle confidential information with discretion and professionalism.

·        Ability to exhibit a professional, positive attitude and work ethic.

·        Ability to be nonjudgmental and show empathy toward distressed individuals

·        Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities.

·        Have a demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g., people of color, LGBTQ+ people, immigrants, justice involved persons, etc.)

·        Strong computer proficiency using standard office software programs required. This includes ability to use technology such as Microsoft Office and web-based applications in daily work.

·        Access to fast, reliable wireless internet without data use limitations at the remote location you will work is required (assistance may be available to develop this access).

·        Laptops will be provided by the program, which will serve multiple functions, including connection to Salesforce, data recording, resource list management and searching, as well as soft phone application. Resource Navigators are expected to use laptops and other technology provided by the program for all work-related functions. A personal mobile device on which a softphone app can be downloaded is strongly encouraged. Resource Navigators will not use their personal phone number for communication at any time.

·        Scheduling availability on some holidays, nights, and weekends
