Description

Responsible for the provisions and coordination of mental and physical health services, mental health counseling, follow-up, home visit, social service referrals, and outreach on behalf of clients of a DOHMH HOPWA Supportive Housing Program. Ensures compliance with contract services requirements, carries a caseload, maintains regular contact with clients, ensure client are connected to care.


Essential Job Functions


The following duties are mandatory requirements of the job:


q Maintain a maximum caseload of 25 single individuals and/or families.

q Provide mental health & supportive counseling

q Provide case management services; including, but not limited to advocacy and counseling, to both the primary client and those identified in the home as collaterals.

q Assessing clients’ needs with particular attention to family dynamics, mental health, and overall aspect of their wellbeing.

q Provide care coordination with medical providers.

q Completion of assessments/reassessments and service plan development.

q Group facilitation in the areas of health promotion; housing readiness; support; mental health; and substance use.

q Provide housing, entitlement, and medical escorts as needed.

q Conduct home visits, hospital visits, and apartment inspections throughout the Bronx and Manhattan.

q Submission of timely monthly documentation.

Other Responsibilities


q Attend divisional housing meetings, weekly staff meetings, quarterly agency meetings, and other internal staff meeting as required.

q Work closely with client’s to encourage compliance with program’s Terms and Conditions agreement.

q Assist client with maintaining housing stability through collection and monitoring of monthly rent payments.

q Appropriate and timely referrals on behalf of clients, provides client advocacy in negotiating systems and delivery of services. 

q Attend and/or coordinate case conferences with other providers.


Education and Certification


q LMSW or MSW required

q A strong working knowledge/ background working with the HIV/AIDS population and services rendered to the HIV/AIDS, mentally ill, chronically homeless, and substance use population and their families. 

q Bilingual French or Spanish preferred, but not required

In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:


q Knowledge of HIV-related issues required.

q Knowledge of HRA/HASA and other entitlements.

q Knowledge of DOHMH HOPWA regulations and eCOMPAS reporting

q  Basic computer literacy (e.g., Microsoft Word, e-mail, internet browsing).

q Crisis management and de-escalation skills.

q Knowledgeable in the use of Motivational Interviewing and Harm Reduction.
