SeniorCare Inc.’s Care Transitions Care Manager (CTCM) provides client-centered assessments to facilitate discharge planning to assist elders to return to the community after a nursing facility (NF) stay. The position involves on-site assessments within SeniorCare’s seven nursing facilities as well as in consumer homes within the nine town catchment area. The CTCM will work with the NF staff to encourage submission of referrals and communication around discharge planning for services needed in the community. Intakes may be completed in a NF, hospital or consumer’s home. The CTCM will do on-site visits to the seven NFs at least monthly. Options counseling for those that are not appropriate for home care services will be offered by the CTCM and familiarity with the process is an integral aspect of the position. CTCM will work closely with SeniorCare’s provider network for service provision needed post discharge, some of which may be immediate. CTCM will carry a small caseload, provide Options Counseling to members of the community and conduct Home Care Intakes as needed.
We are seeking an individual with a minimum of a Bachelor’s degree, preferably in social work or related field, with a social work license or agreement to receive appropriate supervision for licensure. If not already an Options Counselor (OC), EOEA OC training will be required. Effective communication skills are necessary for interaction with the consumer, family, professionals and other supports. Must be able to utilize a computer for data entry, word processing and to research resources. Experience with elders and/or in a community setting is preferred.
