La Clínica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible health care to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care. As health care activists, we are dedicated to making sure individuals who do not have health coverage get the same level of quality healthcare as those who have it. From our genesis as a community health center in 1971, we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. Many times, this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care.
•Participates in the ongoing work of La Clinica to evaluate and manage performance data.
•Assists with coordination and management of programs to help La Clinica improve performance on quality measures.
•In collaboration with the Office of the Medical Director, supports efforts to further develop La Clinica's culture of Population Health Management.
•Coordinates the activities of Panel Management staff, site QI teams, and provides Technical Assistance (TA) and coaching of teams and team members.
•Coaches and advises departments and department leads on QI techniques. Manages and delivers data on QI strategies.
•Assist in the standardization and updating of Panel Management workflows.
•Serves as agency lead for i2i Systems (health registry software), including TA, coordination of user activity on the system, assisting with EHR mapping and ongoing system maintenance.
•Assist in creating/ maintaining platforms for collecting and disseminating QI and data management and sharing best practices.
•In conjunction with relevant IT and clinical leads, coordinates the collection and reporting on QI measures for the facilitation of QI dashboards, tracking health plan Pay for Performance metrics, Uniform Data System (UDS), Patient Centered Health Home (PCHH) and Meaningful Use (MU).
•Supports risk management infrastructure/plan as needed; may include activities such as QA tracking, assisting the Compliance Officer in clinical matters, and completing malpractice redeeming applications.
•Assists in the development of QI policies and procedures, as directed
•Creates and maintains agency, site, and provider level QI reports and dashboards
•Other duties as assigned
•Two (2) years' of experience working with data including analytics. Experience in quality improvement, program development, or chronic disease care highly desirable.
•Experience in public health, community clinics, or other safety net setting highly desirable.
•Proficiency in Microsoft Excel, Word, and Power-point
