Seeking a full time (40 hours/week) care services coordinator. Flexible work from home option is possible for part of the week. Responsible for implementing programs, including information and resource referral and support group facilitation. Primary responsibilities include: conducting in-home assessments, managing equipment loan program, establishing strategic community and professional relationships, providing educational and chapter service outreach, participation in ALS clinics and/or ALS Certified Centers of Excellence.
Qualifications include: Bachelor's degree from 4-year college/university, minimum of 2 years of experience. Case manager or allied health professional preferred. Prior experience with home care, elder care, medical rehabilitation or hospice preferred. Ideal skill set to include empathetic listening skills, end of life issues and a flexible “think outside the box” attitude.
Computer literacy required, ability to travel to constituents as required, ability to work occasional nights/weekends as required.
