The Community Relations & Clinical Assessment Manager, Substance Use Treatment reports to the Program Director. This position is responsible for the overall development and implementation of the community education in regards to signs, symptoms and consequences of having a substance use disorder, the importance of immediate access to respectful care that comprehensively targets the multiple needs of the person, and how to access Thresholds services. In addition, it interfaces with potential referral sources for all community-based Substance Use Treatment Programming, provides excellent and responsive service to prospective clients and their families from initial inquiry through eligibility assessment and connection with the appropriate team.

 

ESSENTIAL DUTIES & RESPONSIBILITIES

Outreach and Community Education

  • Generate the strategic community education plan and carry out such plan by educating those who come in contact with prospective referrals, and promote the Thresholds Substance Use Treatment Services to referral sources to increase program visibility to other professionals.
  • Lead and develop educational and referral materials, including brochures, info sheets and website information, as needed for various stakeholder groups.
  • In collaboration with Team Leaders, Program Director, and senior leadership, reaches out to potential referral sources and community stakeholders, schedules in-person meetings and group forums, and presents educational materials regarding Substance Use Treatment Services. These may include contacts with hospitals, high schools, colleges and universities, primary care physicians, other behavioral health providers, employers, unions and trade groups, employee assistance programs (EAPs), parent organizations and others.
  • Provides monthly follow up with referral sources in order to ensure program exposure and continuity of care.

Intake and Assessment

  • Responds to all inquiries via phone and email within 24 hours.
  • Provides educational information and completes phone screen upon contact.
  • Provides exceptional customer service to clients and referral sources.
  • Provides clarity and direction to referrals on how to manage the complex systems of care in order to access the right services.
  • Completes Initial Screening Tool and provides psychoeducation via in-person meeting with potential client and family within 72 hours at a location convenient to the client.
  • Schedules, completes and scores substance use and integrated mental health assessments as appropriate.
  • Follows protocols regarding intake process and program eligibility as determined by Substance Use Treatment Program leadership.
  • Consults with Program Director regarding diagnostic impression, eligibility, and appropriate team assignment within 24 hours of initial screening.
  • May complete initial IM-CANS assessments and Comprehensive Substance Use Assessments in some circumstances.
  • Provides education regarding available services; facilitates smooth transition to designated Team Leader including providing completed screening and assessment tools.
  • Works in consultation with the Team Leader, Program Director and Senior Leadership to assess for safety and completes safety assessments as needed.
  • Completes all intake documentation in a timely and thorough fashion as per agency and program requirements.

Referral and Resource Guide

  • Creates and maintains database of local services; actively refers those who do not meet Thresholds eligibility requirements or who require supplemental services to appropriate alternative service providers.

Data Management and Tracking

  • Systematically collects and organizes program data pertaining to intake and referral.
  • Produces a monthly data analysis and summary report.
  • Keeps logs and other records of all contacts as required by agency or program practices.

Billing

  • Oversees the coordination of billing, revenue cycle and data management procedures.
  • Connects regularly with revenue cycle department and clinical staff to ensure billing collection, insurance verification, and finance plans for families.

Insurance Screening

  • Initiates process of identifying insurance benefits or other funding mechanisms for potential new clients for both commercial insurance and public payers.
  • Obtains necessary information from referral source and initial client contact; may be responsible for verification of insurance, explanation of benefits, identification of covered and uncovered service components.
  • Assists with insurance enrollment as needed.

Other duties as assigned.

 

MARGINAL DUTIES & RESPONSIBILITIES

  • May be required to work a flexible schedule and be available evenings and weekends.
  • Performs and participates in crisis intervention when necessary.

 

EDUCATION

  • Must meet Office of Mental Health standards as a Licensed Professional in the Healing Arts (LPHA).

 

EXPERIENCE

  • Minimum of five (5) years working with individuals who have severe courses of substance use disorder.
  • Computer skills required; knowledge of database functions and maintenance necessary.

 

SKILLS/CERTIFICATIONS

  • LPHA required (LCPC, LCSW, LMFT), CADC is a plus
  • Must complete and update profiles for the Council for Affordable Quality Healthcare (CAQH) and National Provider Identifier (NPI).
  • Must successfully complete the Substance Use Assessments and IM-CANS certifications.
  • Valid Driver’s License and daily access to a personal vehicle is required. Must obtain and maintain $100,000/$300,000/$100,000 liability insurance coverage. Subject to validation every 6 months.
  • May be required to provide transportation for members of the program using a personal vehicle. Must be 23 years of age and must meet all insurance carrier’s requirements. Subject to annual Department of Motor Vehicles license verification.
