The Youth Program Case Manager is responsible for providing case management, academic guidance, college navigation and support to the Goodwill Youth Programs. Build professional relationships with students and parents and/or guardians that facilitate successful outcomes
Provide case management to assigned students including establishing short and long term goal objectives, monitor their achievement through regular meetings, and identify barriers and steps necessary to overcome obstacles and direct advocacy as needed.
Complete intake assessment including all required forms and documents for student's enrollment.
Maintain clear and accurate case notes of all significant interaction to student files.
Determine whether student needs would most appropriately be met through the provision of direct services utilizing Goodwill resources or by referral to another organization.
Provide services and/or referrals for needs such as housing, health care, childcare, chemical dependency, mental health, legal issues, immigration, domestic violence, budgeting, food, utilities assistance, transportation, further education, etc.
Follow up with providers to whom referrals have been made or that have made a commitment of resources to the student.
Follow up with graduates for one year after employment placement, college placement, and/or High School Credential to track and provide support services.
Maintain orderly and up-to-date files of assigned caseload and meet requirements of Workforce Development Council (WDC)
and Seattle Goodwill data entry and grant reporting.
Present individual cases in the weekly case review meeting and relay all pertinent student information to the Youth Program Manager.
Attend Workforce Development Council (WDC) required meetings and trainings.
Academic Guidance and College Navigation
Monitor academic progress of students and advise students; analyze progress reports from teachers; determine eligibility and satisfactory progress toward their diploma and/or post-secondary credits; identify current and potential needs or problem areas (e.g., study skills, tutoring) and find appropriate resources for the student.
Serve as a liaison between Goodwill, iGrad, and iGrad partners. Communicate each student's plan to their advisor and request student's school progress data. Participate in iGrad meetings, orientations, events.
Assist students to develop college plans based on career interests.
Serve as a liaison between Goodwill and area community colleges, building and maintaining relationships with key individuals in instruction, student services, registration, and financial aid.
Track college enrollments, student status, and credit/credential gains.
Upholds safe work practices in support of a safety culture through awareness and observation; train and educate employees of safety requirements; reports any potential hazards or accidents.
Other duties as assigned.

Minimum Job Requirements
Education: Bachelor's Degree in Education, Counseling, Social Science or related field.
Experience: Minimum 2 years social work, human services or case management experience required. Two years' experience working with high school students , in coordinating partnership with schools, parents and community organizations, data collection and reporting and youth academic program development, or equivalent combination of education and experience. Must have experience working with multi-cultural youth who may have been exposed to poverty, gang violence, and substance abuse.
Specific Skills/Knowledge/Licenses: Bilingual skills a plus. Ability to handle confidential information. Proficient in Microsoft Office and able to learn new software as needed. Must be flexible and have the ability to work in an environment with very diverse group of people.
Must have valid WA State driver's license and clean driving record. Willingness to drive large vehicles (12-15-passenger vans).
Essential Physical Abilities: Able to safely lift/push/ pull up to 25 lbs., grasp, handle, finger and manipulate items; perform repetitive fine motor functions with fingers, wrists and arms, standing for long periods of time. Able to communicate with others effectively, verbally and in writing; including those who have limited English skills, large and small groups, and individuals.

Office/classroom environment; the noise level in the work environment is usually moderate. This position requires some travel to other locations for offsite meetings and events.
