Background:

Based in East Harlem and Washington Heights, two long-standing and well-regarded harm reduction organizations in New York City are seeking a Care Coordination Manager. The non-profits, in the process of completing a merger, are innovative organizations serving low-income people who use drugs, engage in sex work, their families, and their communities, as well as other marginalized individuals. The wide range of mobile and center-based services are delivered by upwards of 80 staff and peer workers, and include syringe access, overdose prevention, public health, care coordination, medical care, holistic care, peer education, and advocacy services. The organizations have a combined $7 million budget made up of mainly government grants. 

Position Summary:

The Washington Heights CORNER Project (WHCP) is seeking a committed, compassionate, and driven individual for a leadership role with our agency. The Care Coordination Manager will supervise all community testing and case management initiatives to ensure staff is providing quality and professional care coordination services and linking clients to services as needed.

Care Coordination Manger will also ensure compliance with state regulatory requirements around HIV and HCV testing and ensure that staff is meeting contract deliverables and capturing client encounters accurately and in a timely manner. This position reports directly to the Director of Clinical and Holistic Health.


Responsibilities:

  • Oversee all care coordination services including the HIV/HCV testing program, supply ordering and inventory, program implementation, staffing, logging client encounters, program policies and procedures, and ensure staff reaches deliverable goals across
  • Supervise the full care coordination team across the organization.
  • Oversee all care coordination services delivered by staff and interns, including intake, assessment, re-assessment, referrals, health services, escorts and linkage to care.
  • Ease participant accessibility to healthcare, social and mental health services as appropriate.
  • Lead training and implementation of systems to promote communication of care coordination services. Oversee documentation of services to ensure provision of care is high quality, audit worthy, and culturally appropriate. Provide consistent supervision to program staff with special attention to engagement strategies, boundary setting, safety, training needs, and knowledge of referral sources.
  • Serve as the primary contact for collaboration with external service providers including health, medical, legal, housing placement, behavioral health, drug treatment, and entitlements services.
  • Collaborate with Program Directors to develop MOUs with partner agencies. Maintain updated contact and referral directories for care coordination team members.
  • Assign cases to case management team and ensure case conferencing and follow up is completed as needed.
  • Coordinate with the Data and Evaluation team members to review data entry procedures, monthly reporting, trends related to drop-in center services and patterns of drug use to inform educational strategies to guide program staff in service provision.
  • Oversee all quality assurance activities related to care coordination, health services, with special attention to contract deliverables.
  • Work in coordination with Program Directors to maintain and revise marketing and promotional materials to be used both internally and externally to promote WHCP services and events.
  • Submit monthly reports on time to director-level staff as needed for contract management, foundation reporting, and communications.
  • Collaborate with the Program Directors to ensure adequate staff coverage on an ongoing basis and modification due to unexpected absence or position vacancy. Assist in facilitation of daily staff meetings and ongoing staff development activities.
  • Perform administrative duties related to payment requests, supply inventory, and internal program materials.


Requirements and Experience:

  • Minimum 5 years of experience working in a relevant field/case management experience in NYC required; Bachelor degree required, Master Degree preferred (LCSW, LMSW, MPH, MPA, MHSA, SIFI)
  • Minimum of 2 years management experience; managing staff in a harm reduction setting a plus
  • Commitment and understanding of the harm reduction philosophy in both service provision and management; familiarity with HIV and HCV service provision, social service agencies and health care services required
  • Strong proven leadership skills a must. Ability to motivate staff, provide constructive and supportive feedback, and adapt approach to match various learning styles and abilities
  • Should be organized, self-motivated, creative, and committed to working in a diverse environment
  • Must be a team player and able to work effectively with a large variety of personalities and abilities
  • Ability to coordinate multiple tasks simultaneously in a fast-paced environment
  • Committed to working with people who use drugs
  • Pragmatic and competent problem solving and conflict resolution skills
  • Ability to work with diverse communities (gender, ethnic, age, language, political)
  • Ability to master a wide range of information and communicate accordingly
  • Excellent communication skills (written and oral); timely follow up on tasks for staff and supervisor
  • Great attitude and people skills are a must
